(404)842-6516 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in Federal Acquisition
Regulations (FAR), subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only Request for Proposals, Solicitation Number 97-62(N). Written
solicitation will not be issued. The procurement is unrestricted. The
standard industrial classification code (SIC) is 3534. The small
business size standard is 500 employees. PERIOD OF PERFORMANCE: The
period of performance will be for one (1) twelve month Base Period and
four (4) twelve month Option Renewal Periods. SCHEDULE OF ITEMS:
001-Maintenance, Repair, Call Back Service and Safety Inspections of
six (6) Otis Traction Elevators plus two (2) Otis Hydraulic Elevators
@ $(enter amt) per month, Total $(enter amt) per year; 002-Maintenance,
Repair, Call Back Service and Safety Inspections of seven (7)
Westinghouse Traction Elevators plus two (2) Westinghouse Traction
Elevators Retrofitted with Montgomery/Kone Controls @ $(enter amt) per
month, Total $(enter amt) per year. The offeror shall submit pricing
in accordance with 001 and 002 above for the base year and each option
period. DESCRIPTION OF REQUIREMENTS: The Contractor shall provide all
labor, parts, materials, supplies and equipment to maintain elevators
in a safe and satisfactory operating condition in accordance with the
current edition of the American Standard Safety Code for Elevators
(ASME 17.1) and the manufacturer s original design tolerances and
specifications. Parts installed on the elevators, either as preventive
maintenance replacement and/or repairs, shall be supplied by or
certified as equivalent by the supplier to manufacturer s original
equipment. In accordance with standard elevator industry practice, the
Contractor shall inspect, maintain, repair, clean and paint all
elevator equipment to ensure peak performance and competent wear
tolerances within original design limits. All signal lenses, bulbs and
push buttons shall be operational and maintained. This contract does
not include repairs or renewals of hoist way enclosures, bodies of
doors and cab interiors. The Contractor shall maintain the machine room
and pit areas clean and remove all trash. All maintenance work shall be
performed during regular work hours (7:00 AM to 5:00 PM). Contractor
will respond within one (1) hour to service calls seven (7) days per
week, 24 hours per day as part of the monthly contract price. All work
shall be performed by skilled elevator personnel directly employed and
supervised by the Contractor. Prior to commencement of the contract,
the Contractor shall submit in writing to the Contracting Officer for
approval a maintenance schedule outlining the periodic maintenance
recommended by the equipment manufacturer for each elevator. Records of
yearly and five year safety inspections shall be furnished to the
Contracting Officer. Government furnished materials include as-built,
up-to-date, wiring diagrams for the Montgomery/Kone retrofitted
elevators only. The Contractor is responsible for obtaining as-built,
up-to-date, wiring diagrams for the remaining elevators. The elevators
are located at the Centers for Disease Control and Prevention (CDC),
Clifton Road Campus, 1600 Clifton Road, Atlanta, Georgia, Building 1
(Main/East/South) and Buildings 2-3-5-6-7. All contract employees are
required to submit a driver s license at the Control Desk, Building 1,
in exchange for a badge pass and key to the elevator machine rooms
prior to each service call. The license will be returned upon surrender
of the badge and keys. In addition, Contractors shall sign-in/sign-out
prior to beginning work and at the completion of the work in Building
1 South, Room SSB256. EVALUATION FACTORS: (1) Past Performance: The
offeror shall demonstrate satisfactory past performance on current or
completed contracts, similar in size and complexity to the requirement
of this RFP. Include name and address of Company or Government Agency,
name and telephone number of customer contact point, contract number
and description of work performed. (2) Price. Technical proposals are
considered to be of more importance than cost or price, however as
technical proposals become more closely scored, cost or price may
become of equal importance. It is the intent of the Government to make
a single award to the responsible offeror whose proposal conforms to
all requirements of the solicitation and is considered to be the most
advantageous to the Government. CONTRACT AWARD: Award of a firm-fixed
price contract will be made on or about June 4, 1997. TERMS AND
CONDITIONS: The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular Number
90-45. FAR provisions at 52.212-1, Instruction to Offerors --
Commercial Items and FAR 52.212-2, Evaluation -- Commercial Items apply
to this acquisition. The offeror is advised that FAR provisions
52-212-4, Contract Terms and Conditions -- Commercial Items and
52.212-5 Contract terms and Conditions Required to Implement Statues or
Executive Orders-Commercial Items apply to this acquisition. Department
of Labor Wage Determination No. 68-0307 (Rev. No. 28), dated 1/23/97,
applies to this solicitation and any resultant contract. Health and
Safety: Prior to commencement of work, each Contractor elevator
mechanic working under this contract shall obtain an initial TB skin
test and shall obtain an annual TB skin test thereafter. The TB skin
test will be provided by the CDC Health Clinic at no cost to the
Contractor. However, if a skin test is positive, the Contractor shall
require the employee to obtain an X-ray at the Contractor s expense.
The results of any X-rays shall be furnished by the Contractor to the
CDC Health Clinic. Security Clearances: All contract employees
performing work at CDC facilities are required to obtain a National
Agency Clearance (NACI). All paperwork for the NACI shall be completed
by each employee prior to performance of work. As part of the initial
paperwork, each employee will be required to submit a GCIC. A
temporary clearance will be issued for each employee to allow for
performance of the work while the NACI is being obtained. The cost of
the GCIC and any DMV reports required shall be the responsibility of
the Contractor. The Government reserves the right to negotiate with
offerors to modify the terms and conditions to meet the needs of the
Government. PROPOSAL SUBMITTALS: Submit the following to the Centers
for Disease Control and Prevention, PGO, Attn: B. A. Blackledge, 255 E.
Paces Ferry Road, NW, Room 419, Atlanta, GA 30305 (1) an original and
three copies of a completed SF-1449, signed by an official authorized
to bind the organization; (2) Completed Representations and
Certification-Commercial Items; (3) Data required under Evaluation
Factors; (4) Acknowledgment of solicitation amendments, if any; and (5)
Offeror s Standard Pricing Agreement and Standard Commercial Terms and
Conditions. (6) List of names, titles and qualifications of proposed
personnel working on contract. Offers that fail to furnish the required
information or reject the terms and conditions of the solicitation may
be excluded from consideration. DUE DATE: Proposals are due on May 5,
