This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 626-38-97 is issued as an invitation to bid (IFB). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. The Standard Industrial Classification Code is 2834. The following items are to be delivered FOB destination within consignee's premises, in such quantities and at such times as may be required in accordance with all terms, conditions, provisions, and the schedule of this solicitation, for the period 5/1/97 through 9/30/97: Photopheresis Procedural Kit equal to Therakos TK001, used in the treatment of Cutaneous T-Cell Lymphoma; estimated quantity, 72 cases, 15/case Provide Photopheresis Procedural Kits for the VA Medical Center, Nashville, TN. Kits shall be compatible with the UVAR Photopheresis Systems manufactured by Therakos, Inc. The supplies to be furnished under this contract shall be ordered by issuance of delivery orders. Such orders may be issued from 5/1/97 through 9/30/97. Supplies shall be delivered FOB destination to Chief, A&MMS (90), VA Medical Center, 1310 24th Ave., S., Nashville, TN 37212. If other than brand name identified is offered, then the offeror shall indicate the manufcturer, brand, model/part number and a description in sufficient detail to show that the product offered meets the brand name identified.
