This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. The James A. Haley VA Hospital intends to purchase the following items based on Chattanooga Group, Inc. brand name or equal: 1) Hydrocollator Colpac Chilling Unit, Model C-6 #3202, Qty. 1 each, to include optional accessory #4010-side table rack 2) Hydrocollator Mobile Heating Unit, Model M-4 #2502, Qty. 3 each, to include optional accessory #4010-side table rack 3) Intersegmental Traction Table, Model 8770, Qty. 1 each 4) Ultrasonic treatment unit, mobile, Model CPS004, Forte 400 Combo, Qty. 5 each 5) Traction table, model Triton TRE-43 #7730, Qty. 3 each 6) Traction Unit, model TX-1 #7101, Qty. 3 each, to include optional accessories a) #7040-saunders cervical traction system, b) #1440-heavy duty pelvic traction system, c) #7050 TRS-30 flexion stool. All items must include 2 copies of MFG. Technical literature to include operator's manuals, service/calibration manuals and illustrated parts breakdown for each item. Any brand name equal product that is submitted for review must include a picture of the item and adequate technical information to allow the Government to review it, otherwise the brand name equal submission shall be disallowed. Quotations shall be submitted on an "all or none" basis. Partial quotations shall be rejected. All quotations shall state Delivery time, F.O.B. point, shipping weight and cost if any. Offerors shall provide a complete set of provisions with quotation found at 52.212-3, Offeror Representation and Certification-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply. Quotations shall be submitted to the address listed herein or fax to 813-903-4838 and directed to the attention of Eric Guidash (90C).
