Traxler Part I of II. This is a combined synopsis/solicitation for
commercial services prepared in accordance with the format in Subpart
12.6, as supplemented with additional information in this notice. This
is a Request For Quotation (RFQ). Responses are due on or before May
13, 1997, 4:30 local time at the VA Medical Center, ATTN: Contracting
Officer (90C), 1011 Honor Heights Drive, Muskogee, OK 74401. PROVIDE
valet parking, in a designated area, for a percentage of approximately
200 outpatient visits per day. Clinic hours are 8:00am-4:30pm, Monday
through Friday excluding ten (10) federal holidays. Contract period is
from date of award through September 30, 1997. Please offer a quote per
attendant/per hour $__ __ x 1,920 hours = $__ ___ Total Price
REQUIREMENTS: The contractor will park all vehicles reporting to the
valet operations. The contractor will operate from a VA provided,
non-conditioned booth located near the main entrance. The contractor
will greet every vehicle dropping off or picking up patients and their
families and offer valet parking. All contract attendants will be
polite and respectful to all customers. Upon return, parking patrons
will have car delivered to front entrance by attendants. The VA will
provide a two-way radio for communication with VA police. The
contractor will be responsible for all charges associated with
communication with their office. The contractor is responsible for
parking control and monitoring in the designated parking area,
responding to alerts or emergencies by providing assistance to injured
or stranded persons in emergency situation and by notifying the VA
Police. After normal operating hours, the contractor will turn over
keys of any vehicles still on premises to the VA Police. Owners of
these vehicles will pick up their keys at the VA Police office. The
contractor will furnish a 3-part claim check, cones, or any other
miscellaneous items necessary to facilitate valet parking. The
contractor will provide uniforms to all personnel. Uniforms will
include identification of the company and employee. Uniforms must be
worn at all times. All uniform styles must be approved by the
Contracting Officer's Technical Representative prior to implementation.
The contractor will provide adequate on-site supervision at all times
to handle customer concerns and parking incidents. The supervisor must
be a working supervisor. STAFFING: Starting staffing levels will be
agreed upon during a post-award conference. The contractor will utilize
the appropriate level of staffing to ensure efficient operation.
Efficient operation is defined as: No vehicle waits more than five
minutes to be parked or returned, 95% of the time. The COTR will audit
the turn around time. Vehicles will be stopped and the driver will be
asked for PATIENT IDENTIFICATION. Valet parking will also be offered
to visitors. The visitor will be asked the patient's name. The
contractor will notify the VA Police of handicapped or injured patients
requiringassistance. Driver's keys will be secured. This service is for
PATIENTS AND THEIR VISITORS ONLY. NO EMPLOYEES will be authorized to
utilize this service. CUSTOMER SERVICE: All parking attendants will be
trained in demonstrating courtesy, kindness, and caring. A NO TIPPING
sign will be furnished and displayed by the contractor. No tipping
will be strictly enforced. TRAINING/LICENSES/LIABILITY: The VA will
provide orientation and training on VA policies and procedures. This
training will be scheduled at the post-award conference. The contractor
is required to have security and background checks on all employees
working under this contract. ALL ATTENDANTS MUST HAVE A VALID STATE OF
OKLAHOMA DRIVER'S LICENSE and be U.S. citizens. Contractor must have
liability insurance as required by the state to operate a valet
service. Proof of insurance must be submitted with the bid. CLAIMS AND
DAMAGES: Any claims of damage or missing/stolen property involving a
customer's vehicle will be the responsibility of the contractor.
