Q -- PRIMARY CARE/FOLLOW UP SERVICES SOL RFP 635-50-97 DUE 063097 POC Beverly Kimball, Contracting Officer This VA medical center has a requirement for utilization of a community-based clinic with physicians and personnel in the area of Wichita Falls, TX for primary care and routine/follow up services to include normal x-ray, laboratory, and pharmaceutical services. Offerors wishing to submit proposals for clinic space only are also encouraged to respond and will have the opportunity to bid a separate option for this purpose. Clinic must be located within a 30 mile radius from the center of the Wichita Falls, TX area and must be located within this medical center's primary service area. Physicians and technicians operating under the resulting contract must meet all evaluation criteria and other professional certifications required to operate in such a clinic. The qualifications of such personnel shall also be subject to review by the Chief of Staff of this medical center and approved the Oklahoma City VA Medical Center Director. Clinic profiles will be established as clinic staff deems appropriate to accommodate workload demand. Solicitation issue date shall be on or about June 30, 1997.
