The VA Medical Center, West Los Angeles, CA, intends to negotiate a Sole Source contract with Siemens Nuclear for Preventive Maintenance of Siemens PET Scanner Model #ECAT 95 3/31, Catalog #5551858, located in Building 304, Room 200, Nuclear Medicine Department. This is a combined synopsis/solicitation for commercial items prepared in attendance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation #691-53-97. The solicitation is issued as Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #90-46. The procurement is unrestricted. The associated standard industrial classification code is #7629. The small business size standard is $5.0 millions of dollars. PERIOD OF PERFORMANCE: The period of performance will be from Date of Award through 9/30/97, with two (2) Option Renewal Years. SCHEDULE OF ITEM: 1-Preventive Maintenance Siemens Pet Scanner Model #ECAT 95 3/31, Catalog #5551858 @ $(enter amount) per month, Total $(enter amount) per year. Offeror shall submit pricing in accordance with item #1 above for the base period and each option year. DESCRIPTION OF REQUIREMENTS: All work must be conducted in accordance with Siemens Nuclear service manual and VA Medical Center regulations approved repair department. The contractor will furnish all necessary labor, tools, materials, supplies/parts, manuals and schematic drawings necessary to perform scheduled preventive maintenance service, emergency repair work and performance evaluations on the listed medical equipment. Contractor shall include all major and minor parts/supplies necessary for repair and maintenance. Contractor shall have access and readily available manufacturer's replacement parts. PRE-INSPECTION: Equipment placed under this service contract shall be in good operating condition. SCHEDULED PREVENTIVE MAINTENANCE INSPECTION/SERVICE: Preventive maintenance will be done during normal working hours, Monday through Friday, 8:00am to 5:00pm, excluding Federal Holidays. Overtime work of preventive maintenance service outside of normal working hours, after 5:00pm, and before 8:00am Monday through Friday, any Saturday, Sunday, or Federal Holiday) will require the prior approval by the Contracting Officer's Technical Representative (COTR) through the Contracting Officer. The Contractor will contact the COTR to arrange for all services on this station at (310) 478-3711 (x40089) or (x49187). Scheduled preventive maintenance cycles must be completed within one working day, extensions beyond one working day require the approval of the unit supervisor and Chief, Biomedical Engineering. EMERGENCY REPAIR SERVICE: Unlimited emergency repair service to be available on site when necessary, Monday through Friday during normal business hours. The Contractor shall maintain and operate a telephone emergency assistance number, available 24 hours/day. The telephone response time for the contractor, after the initial call, shall be one (1) hour. The contractor's service personnel shall respond within one (1) hour to telephone request for emergency service to determine the scope of the problem and make accommodations including scheduling for on-site repairs. Should it be determined that on-site repairs are required, the contractor, must arrive the same working day to start repairs when the initial call is made before noon, if the contractor is initially contacted after 12 noon, then the contractor must make every effort to arrive during the same working day, or no later than start of the next working day. The contractor will contact or be contacted by the Biomedical Engineer, Building 500 Room 0261 at (310) 478-3711(x40089 or x49187) to arrange all services, in additionto signing in, the contractor personnel will identify himself to Biomedical Administrator/technician before proceeding to the job site. MAINTENANCE AND REPAIR SPECIFICATIONS: For each equipment system identified herein the contractor will perform at a minimum those services as determined necessary by the Biomedical engineer and Using Service Supervisor. The Using Service Specification on the listed medical equipment is based on (1) The equipment manufacturers recommended service and preventive maintenance procedures and schedules (2) The medical utilization of the instrument. Performance verification will be required at the conclusion of every preventive maintenance and emergency repair event. This verification procedure may include performance testing for accuracy and precision of the equipment. If any procedures were were performed that would in any way affect the calibration status of the equipment the results of performance testing for accuracy and precision of the equipment. If any procedures were performed that would in any way affect the calibration status of the equipment the results of performance testing must conform to the established performance criteria for the instrument. Preventive maintenance schedule (PMI) shall be on a semi-annual basis. PMI visits must not be less than 180 days apart. Each maintenance or emergency repair event will include inspection for internal and/or external causes of medical and electronic problems, including: (1) wear, misalignment, maladjustment, damage or other malfunctions with respect to the manufactures specifications (2) Inspection for electrical safety and fire hazards (3) Cleaning and lubrication of equipment as required (4) Repair or replacement of all defective components necessary to conform to instrument specifications (5) Calibration or standardization of instrument as required (6) Testing for proper operation of entire system and subsections (7) Document proper function and performance of instrument. Service Technician must document on both the field service report and the instrument maintenance log, all findings of problems and their correction, including all performance of verification. Reports must be legible. Service Technician shall document and report to the Using Service and Biomedical Service and Engineering any unsafe condition concerning this medical equipment or signs of misuse or negligence. AVAILABILITY OF REPAIR EQUIPMENT, SPARE PARTS AND SUPPLIES: (1) Contractor will have necessary equipment, parts and supplies on site within eight (8) hours after initial emergency service call is placed. For scheduled maintenance service, the contractor must have all necessary equipment, parts and supplies on site at the start of the service call. (2) Any operating supplies necessary to perform service or repairs, will be provided by the contractor at no cost to the VA Medical Center. Should the contractor need to use VA materials to effect emergency repairs, these materials will be recorded on the field service report and replaced without cost within (30) days from completion of the service call. (3) Only new standards parts shall be furnished by the contractor in effecting repairs. Parts supplied by the contractor, or in special cases by the Biomedical Engineer, will be installed by the contractor. (4) Parts that must be replaced for maintenance objectives and emergency repairs shall carry standard commercial warranties of (90) days warranty. (5) All parts replaced will be identified by part number as shown on the manufacturers schematics/parts list. Actual cost of parts will be placed on service report but will be a no charge item on contractor's invoice. All parts replaced, shall be new/exchange replacement parts, as suggested by the original manufacturer. PERFORMANCE SPECIFICATIONS: If performance does not conform to the contract specifications, the VA Medical Center will require the contractor to perform the service again in conform to contract specifications, at no increase in the contract cost. When the defects in service cannot be corrected by re-performance, the VA Medical Center may (1) require the contractor to call in a certified technical expert on the list instrument (2) specifically require the contractor call in a senior technical engineer from the manufacturer. Repeated malfunctions of any system component or subsection more than five times within one month will not be tolerated. The contractor is responsible for repairing all faulty equipment and identifying the nature and cause of repeated failures. The contractor in this situation must either: (1) make repairs such that the identified problems are corrected to the extent that repeated failures are eliminated. (2) Prove that the equipment is not faulty and that an external cause for the problem exists with recommendations for resolution. (3) Hire an outside expertly, with Biomedical Engineering's prior approval and at the contractor's expense, to make all the necessary diagnosis and repairs. SERVICE REPORTS: After each service call, a service report shall be written out to include (1) Date and Total hours of service (2) Parts installed (parts#'s and part costs) (3) Safety check (4) Performance data(5) service comments or remarks. This service report shall be signed by the serviceman and unit supervisor. A record of this report must be filed in the designated department service maintenance logbook. The original service report must be delivered to Biomedical Engineering, Building 500, Room 0261. Final signature will be withheld on any invoice where there is no service report to verify that the work was performed in a satisfactory manner, and signed off correctly. All reports must be legible. PERIODIC INSPECTIONS: The VA Medical Center will at its option request an unrelated third party to inspect the quality of service of the contractor. Any faults or omissions discovered by the inspection will be repaired or otherwise corrected at the contractor's own expense. The provision at 52.212-1, Instructions to Offerors -- -Commercial, applies to this acquisition. Provision at 52.212-2, Evaluation-Commercial Items, will be applicability. The Government will award the contract resulting from this solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1)Price (2)Past Performance: The offeror shall demonstrate satisfactory past performance on current or completed contracts, similar in size and complexity to the requirement of this RFP. Include name and address of Company or Government Agency, name and telephone number of customer contact point, contract number and description of work performed. Evaluation factors are in their relative order of importance for the Evaluation factors, such as in descending order of importance. Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212.4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
