synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. The solicitation # is 459-97-0012.
This is a request for quotation, (RFQ). The solicitation document and
incorporated provisions and clauses are in effect through Federal
Acquisition Circular 90-46. (initial year of service), August 1, 1997
thru July 31, 1998 -- DESCRIPTION: 2-six cubic yard containers with
automatic locking lids, pick-up will be 3 times a week, stinger truck
will be required. Quantity: 12, Unit: month, Price $__ ____, Total $__

above, butfor the period August 1, 1998 thru July 31, 1999 and August
1, 1999 thru July 31, 2000 respectively. REQUIREMENTS: Contractor is to
furnish all labor and equipment to provide trash/refuse removal at the
Department of Veterans Affairs, Center for the Aging, Building 110,
110 Krukowski Road, Honolulu, HI. 96819, on the grounds of Tripler Army
Medical Center, Honolulu Hawaii. Contractor is to provide two (2) six
cubic yard containers with automatic locking lids at building 110
loading dock at all times, containers should be in place by August 1,
1997. Contractor will ensure the containers are rust free, neatly
painted, labeled as required and maintains a presentable appearance at
all times. Containers that are considered unacceptable by the
Contracting Officer's Technical Representative shall be replaced by the
contractor at no additional cost to the Government. Contractor will be
required to empty the containers three (3) times a week Monday,
Wednesday, and Friday including all holidays. The containers must be
emptied only between the hours of 10:00am and 3:00pm. If refuse is
dumped on the ground during pick-up the contractor will be responsible
to pick-up this refuse at own expense. Containers will be required to
be moved from the loading dock with a stinger truck for emptying.
Contractor shall ensure they are in full compliance with all state and
local codes when disposing of all refuse. Contractor will ensure all
safety requirements are followed when servicing the containers. Any
safety related discrepancies or hazards noted by the contractor when
servicing should be corrected immediately or reported to the VA for
correction. Contractor will provide a 24 hour emergency number.
Emergency service shall be provided within two (2) hours of
notification. Provision 52.212-1 INSTRUCTIONS TO OFFERS-COMMERCIAL
ITEMS,- COMMERCIAL ITEMS IS APPLICABLE. The Government will evaluate
offers for award purposes by adding the total price for all options to
the total price for the basic requirement. The Government may
determine that an offer is unacceptable if the option prices are
significantly unbalanced. Evaluation of options shall not obligate the
government to exercise the options(s). Offeror must submit a completed
copy of provision 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS
-- COMMERCIAL ITEMS with its quotation. Clause 52.212-4, CONTRACT
TERMS AND CONDITIONS is applicable to this acquisition. Clause
52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES
OR EXECUTIVE ORDERS -- COMMERCIAL ITEMS, applies to this acquisition.
In addition, all applicable clauses and provisions from the VA
Acquisition Regulation apply. Quotations are due to the Contracting
