ABALOS, 619/532-8084, Contracting Officer, MIKE NYE, 619/532-8088 This notice is provided on a sole source basis, with intent to award a firm fixed price order with JOINT COMMISSION ON ACCREDITATION OF HEALTHCARE ORGANIZATIONS (JCAHO). This is a combined synopsis/solicitation for commercial items, prepared in accordance with FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation, number N00259-97-Q-0079, is issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 90-44. Service requested is a customized education course titled HOSPITAL SURVEY PREPARATION CLINIC -- THE ADVANCED COURSE to be conducted for approximately 200 attendees. It will be held for a period of three (3) days, from 22 Sep 97 thru 24 Sep 97. Naval Medical Center San Diego will be responsible for providing the site location. Contractor is to provide course materials, one (1) master print, supplementary handouts, certificates, faculty travel, time and maintenance. This acquisition incorporates the following FAR clauses: 52.212-1 INSTRUCTIONS TO OFFERORS- COMMERCIAL ITEMS (Oct 95); 52.212-2 EVALUATION-COMMERCIAL ITEMS (Oct 95) is incorporated by reference with the following addenda: in paragraph (a) the government will evaluate proposals based on technical capability and cost to the government. All other factors when combined, make up 50 percent of evaluation factors as compared to price; 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS (Oct 95) must be completed and submitted along with the proposal (a copy can be obtained by contacting Glenda C.
