commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation # is 459-97-0013. This is a request for quotation, (RFQ).
The solicitation document and incorporated provisions and clauses are
in effect through Federal Acquisition Circular 90-46. (initial year of
service), July 1, 1997 thru June 30, 1998 -- DESCRIPTION: One 300
gallon bulk liquid oxygen tank, six H Type cylinders,
filling/maintenance service be required. 1. 300Gal Tank Rental/
Maintenance, Quantity: 12, Unit: Month, Price: $__ ____, 2. Six H Type
Cylinder Tanks Rental, Quantity: 12, Unit: Month, Price: $__ ____, 3.
Liquid Oxygen (estimated usage per year 510,000SCF), Quantity:
510,000SCF, Unit: 100SCF, Price: $__ __ __.. There are options for four
additional years of service. Offers, Please submit pricing for optional
years as well. Items are same as above, but for the period July 1, 1998
thru June 30, 1999, July1, 1999 thru June 30, 2000, July 1, 2000 thru
June 30, 2001 and July 1, 2001 thru June 30, 2002 respectively.
REQUIREMENTS: Contractor is to furnish by lease, one 300 gallon bulk
oxygen tank with gauges, piping, valves, vaporizer, low liquid level
sensor switch and filling/maintenance service to the Department of
Veterans Affairs Center for Aging, Building 110, 110 Krukowski Road,
Honolulu, Hawaii 96819. The 300 gallon tank is to be located on the
south side of the building site. Installation is to be coordinated with
the General Contractor for the Center for the Aging, Metric
Construction Company. All costs associated with installation and
testing shall be incurred by the General Contractor, Metric
Construction Co. All costs for liquid oxygen used for testing and
filling will be borne by the Department of Veterans Affairs. Scheduling
of filling and testing shall be coordinated between the Contractor,
Metric Construction and the Department of Veterans Affairs, Honolulu,
Hawaii. Contractor shall also furnish by lease, six (6) H Type standard
cylinders (approx. 244 scf each) for emergency back-up system. Delivery
shall be to room B101A, Oxygen Storage. Installation and testing cost
will be the responsibility of Metric Construction Co. Initial delivery
to be scheduled with Metric Construction Co. and the Department of
Veterans Affairs, Honolulu, Hawaii. Contractor is to ensure both the
bulk tank and the H Type cylinders are available for Metric
Construction Co. to complete all certifications and testing by July 18,
1997. Contractor will be responsible for all maintenance, inspections,
certifications and repairs for components mounted to and including the
bulk oxygen storage tank, after the initial installation, testing and
certification by Metric Construction Co. These components include;
valves, gauges, piping, and vaporizer. The contractor will not be
responsible for components downstream of the vaporizer. Contractor is
to ensure the tank is rust free, painted, labeled as required and
maintains a presentable appearance at all times. Contractor will ensure
that all equipment, labor, servicing, maintenance, testing, and
certifications comply with the following regulations: NFPA 99, 1996ED
Standards For Health Care Facilities Chapter 4, Gas and Vacuum Systems
and NFPA 50, 1996ED Bulk Oxygen System, Chapters 3, 4, and 5.
Contractor will ensure tank filling is provided on a regular basis so
that the capacity is never below one third. All safety
requirements/notifications will be posted when servicing the tank. Any
safety related discrepancies or hazards noted by the contractor when
servicing should be corrected immediately or reported to the VA.
Contractor will provide a 24 hour emergency number. Emergency service
shall be provided within two (2) hours of notification. . Provision
52.212-1 INSTRUCTIONS TO OFFERS-COMMERCIAL ITEMS,- COMMERCIAL ITEMS IS
APPLICABLE. The Government will evaluate offers for award purposes by
adding the total price for all options to the total price for the
basic requirement. The Government may determine that an offer is
unacceptable if the option prices are significantly unbalanced.
Evaluation of options shall not obligate the government to exercise the
options(s). Offeror must submit a completed copy of provision 52.212-3,
OFFEROR REPRESENTATIONS AND CERTIFICATIONS -- COMMERCIAL ITEMS with its
quotation. Clause 52.212-4, CONTRACT TERMS AND CONDITIONS is applicable
to this acquisition. Clause 52.212-5, CONTRACT TERMS AND CONDITIONS
REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS -- COMMERCIAL ITEMS,
applies to this acquisition. In addition, all applicable clauses and
provisions from the VA Acquisition Regulation apply. Quotations are due
to the Contracting officer (90C), 300 Ala Moana Blvd., Room 3119A,
Honolulu, HI 96813, no later than 2:00PM local time May 29, 1997.
