Q -- EMPLOYEE ASSISTANCE PROGRAM SOL RFP 646-45-97 DUE 061797 POC Lynne Dickerhoff, Contract Specialist The VA Pittsburgh Health Care System, University Drive, Pittsburgh, PA 15240 is seeking qualified healthcare providers capable of performing services for the "employee assistance program" in the Pittsburgh, PA geographic area. It is contemplated that a single award will be made to a healthcare facility qualified to provide employee assistance such as, but not limited to assessment, counseling and referral services to employees of the VA Pittsburgh Health Care System and their family members for the purpose of determining the early recognition, intervention, and resolution of work-related and personal problems in accordance with contract terms and conditions from date of award through September 30, 1997, with two (2) one-year Government renewal options. Offeror must have at least three (3) sites in Allegheny County. The Government estimates the workload to be 2600 employees with each employee being seen an average of three (3) one hour sessions. The offeror will quote a capitation rate for the base yearand each option year. Family members will be seen a maximum of three (3) one hour sessions. The offeror will quote an hourly rate for family members for the base year and each option year. The estimated issue date for this solicitation is 5/27/97 and the estimated due date for proposals is 6/17/97.
