Contractor MUST BE located in Sheridan County in order to provide 24 hour emergency service. Optometry services to include but not limted to primary and emergency eye care, advise physicians through phone consultations, in services, and special meetings, any patient rechecks or first follow-ups, one full clinic day at the VAMC Sheridan with any additional clinic days as needed in order to keep waiting times below 30 days. Emergencies to be seen at the VAMC or the contractor's offices at any time. Optometrist on call 24 hours/day 365 days a year for emergencies. Contractor will participate in the implemenetation of effective risk management review, quality assurance, and other accreditation requirements. The contractor will also work with the Chief of Staff and the Chief of Medicine Service to develop efficient operating policies and procedures. SIC 8042. Small business size standard is $5.0 million.
