This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotes (RFQ) are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. Contractor shall provide one Pneumatic Lithotripter System including control unit, 1.8mm hospital grade power supply cable; quick connector with air tube; connector without air tube; 2mm pneumatic foot control with tubing; two handpieces; seals for handpieces; replacement silicone tube; silicone tube with 2mm quick connector; sterilization cap; rigid probes in the following sizes: .8mm, 2 ea, 1.0mm, 2 ea, 1.6mm, 2 ea, 2.0mm, 2 ea.; and a one year warranty. System must use ballistic energy to fragment ureteral and bladder stones. Lithotripsy must be performed under direct endoscopic vision allowing for continuous visualization of the fragmentation process and constant monitoring of contact between probe and stone. Offerors are requested to provide their most current catalogue price. This will be a firm fixed price contract. FOB is Destination. All quotations shall state delivery time. Payment will be made upon receipt and acceptance of the equipment. System shall be located at the VAMC Asheville, NC.
