items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. Solicitation
RFQ 22-CM-97 is issued as a Request For Quotation (RFQ). This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 90-46. SIC Code is 5047
and the small business size is 500 employees. Vendor shall provide
Sorin Biomedical Inc., Blood Cardiplegia BCD Set #BCD VAN8S, or Equal.
Specifications of item referenced is provided for information: Blood:
crystalloid Ratio 8:1, low priming volume, bridged to allow only blood
administration, dual crystalloid solution lines and bag spikes, table
line separately packaged, and integrated temperature port, bubble
trap/air purge, overpressure safety valve, screen filter, and pressure
monitor port with separator. Usage is estimated at 30 per month.
Proposal should include 3 sets Heart-Lung machine tubing inserts and 3
sets of all necessary hardware. Contract period will be from date of
award through September 30, 1997 with 2 one year options to extend the
term of the contract. Bid Sample plus descriptive literature is
required and is to be submitted prior to bid opening to the Contracting
Officer. Deliver FOB Destination. Proposal should address two delivery
options (1) Monthly drop shipments paid by Government Purchase Card
(VISA), (2) Prime Vendor distribution system with Bergen Brunswig our
current prime vendor. Product is for VA Medical Center, Warehouse
(90D), One Veterans Drive, Minneapolis, MN 55417. The solicitation

Availability of Funds for the Next Fiscal Year, 852-210-77 Brand Name
or Equal. Include a completed copy of 52.212-3 Offeror Representation
and Certifications Commercial Items with offer. Evaluations may be
conducted to determine that product offered will meet Government's
needs. Evaluation Factors include technical capability of the item
offered, past performance history, evaluation process and price. Award
of Best Value for the Government contract will be based on the
evaluation factors and will be made with or without discussions and
will be subject to the availability of funds. Offers are due July 11,
