synopsis/solicitation for commercial item/services prepared in
accordance with format in Subparts 12.6 and 13.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation to be issued, proposals are being
requested from this announcement, a written solicitation will not be
issued. The Request for Quote number is 667-97-3-055-0000. This
solicitation incorporate provisions and clauses in effect through FAC
46. Standard Industrialization Code (SIC) of 8011 is applied. This
solicitation is issued subject to the availability of government funds
at time of award. This solicitation is unrestricted for the following
service: Contractor shall provide a board certified orthopedic
physician, proficient in the diagnosis and treatment of patients
referred to by specialty clinics for orthopedic condition. Services
shall be provided at the Overton Brooks VA Medical Center, 510 East
Stoner Ave., Shreveport, LA 71101. Hours of serviceswill be 8 hours per
day as assigned. Period of performance from Mid July to Mid September
1997. The contract physician shall be required to provide consultations
for patients requiring care in specialty clinics as referred by their
primary care physicians and or from other specialties throughout the
VAMC. Procedural services may include those usually performed in a
surgical clinic, as well as others deemed necessary by the Chief of
Staff or designee. Consultation evaluations will be for patient
referrals from physicians practicing at the VAMC, Shreveport, LA with
the intention of discharge and return to the care of the primary care
physician at the earliest appropriate time. Performance of orthopedic
surgical procedures may be required. Estimated number of patients
scheduled per clinic day 10-25 patients. The contract physician will be
expected to prescribe medications for patients according to the
established drug formulary at this facility. Variance from the
formulary procedure should occur only whenclinically indicated. The
contract physician will be required to provide technical guidance,
coordinate and oversee the activities of other clinical personnel
within the orthopedic surgical section. The contract physician will be
required to provide limited on call services. The contract physician
will be required to complete all reports for inclusion into the
patient's record on the day of the visit and shall satisfy VHA and
medical center policies and JCAHO requirements for documentation.
Contractor will act in compliance with all rules and regulations of the
Shreveport VA Medical Center and under the direction of the Chief of
Surgery. Referral and/or transfer of patients to another medical
facility will be completed by VA personnel and approved by Chief of
Staff or designee prior to transfer. A record keeping system of
contractor hours worked will be developed by the Chief, Surgery or
designee. The physician must sign-in before commencing work and
sign-out upon completion of work daily. Payment will be made in
arrears, on a unit basis for services rendered at the prices agreed to.
The contractor is responsible for leave, including sick leave, holiday
or vacation time, taxes and benefits. The following provisions and
clauses are applicable and are hereby incorporated into the
solicitation by reference and will be provided by the contracting
officer in full text upon request: FAR 52.212-1, Instructions to
Offerors Commercial Items applies. Addendum clause FAR 52.215-18,
Facsimile Proposals also applies. Offerors are required to complete and
include a copy of the following provisions with their proposals: FAR
52.212-3, Offeror Representation and certifications Commercial Items,
FAR 52.212-4, Contract terms and Conditions Commercial Items, FAR
52.211-5. FAR 52.212-5, Contract Terms and Conditions Required to
Implement Statutes or Executive Orders Commercial Items applies with
the following applicable clauses for paragraph (b): FAR 52.222-26,
Equal Opportunity, FAR 52.222-35, Affirmative Action for Special
Disabled and Vietnam Era Veterans; FAR 52.222-36, Affirmative Action
for Handicapped Workers; FAR 52.222-37, Employment Reports on Special
Disabled Veterans and Veterans of the Vietnam Era. Award will be made
to that offeror (1 ) whose proposal is technically acceptable and (2)
whose technical/cost relationship is the most advantageous to the
Government. While cost is secondary to technical, it will be a factor
in the award decision. The critical factor in making any cost/technical
trade-off is not the spread between the technical scores, but rather
what is the significance of that difference. The significance of the
spread scores will be determined on the basis of what that difference
might mean in terms of performance and what it would cost the
Government to take advantage of it. Award may not necessarily be made
to the offeror submitting the lowest offer. Also, award may not
necessarily be made for technical capabilities that would appear to
exceed those needed for the successful performance of the work. The
Government reserves the right to make cost/technical tradeoffs that are
in the best interest and to the advantage of the Government.
