C -- WARD RENOVATION PLAN -- WADE PARK SOL 541-181-97 DUE 080197 POC Judith Wilson, (216) 838-6091 The Architect/Engineer shall provide a complete facility development plan for above mentioned project at the Department of Veterans Affairs Medical Center, Wade Park Unit, 10701 East Boulevard, Cleveland, Ohio 44106. A/E shall have principal qualifications in design and facility development planning, general building remodeling including hospital facilities, and also experience in designing for energy conservation, building systems including electrical, plumbing, HVAC mechanical and structural, JCAHO requirements and Life Safety Code. This plan will provide means by which the Cleveland VA Medical Center may more efficiently utilize its space at the Wade Park Facility. The following factors will need to be considered, but not limited in this plan: closing of wards; existing planned projects; the need to consolidate ambulatory care on the first floor; and contigouous space and adjacent space issues. A/E firms which meet the requirements in the announcement shall submit two (2) completed 254's and two (2)completed 255's. Only forms received from firms within a 100 mile radius of the Cleveland VA Medical Center will be accepted. Responses must be received no later than 2PM on August 1, 1997. Selection will be made using the following evaluation factors, respective of scope identified above: (1) Team proposed for this project; (2) Previous experience of the team proposed for this project; (3) Proposed design approach for this project; (4) Location and facilities of working office; (5) Cost control; (6) Estimating effectiveness; and (7) Schedule control. This is not a request for proposal and is unrestricted as to business size. Award of contract is contingent upon recipt of funding for this project, a 90 day bid acceptance period is required.
