Pridmore, Contracting Officer (510) 372-2046 Contractor to furnish Skilled Home Health Care, Bowel and Bladder Care and Homemaker/Home Health Aide Services to patients referred by the VANCHCS residing in the following Northern California Regions: A) Alameda, Contra Costa, Solano Counties; B) Sacramento and Yolo Counties; C) Sutter and Yuba Counties; D) Colusa, Glenn and Lake Counties; E) Butte County; F) Tehama and Trinity Counties; G) Shasta County, and H) Siskiyou County. The Contractor must be certified by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO). The government reserves the right to make one or more contract awards, however only one contract will be awarded for each Region. The contract(s) shall be for a period of one year from date of award with two one-year options for renewal. The anticipated contract award date is 9/1/97.
