Pomraning (410) 605-7156 The Veterans Administration, 10 North Greene
Street, Baltimore, MD 21201, intends to award a service contract for a
Loose Note Filing Project. Request for Proposal (RFP) No. 512(B)-15-97
is a combined synopsis/solicitation for commercial items prepared in
accordance with Subpart 12.6, as supplemented with additional
information included in this notice. This annoucement constitutes the
only solicitation. The solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 90-46. This proposed contract is 100% set aside for small
business concerns. The Standard Industrial Classification is 8744,
Facilities Support Management Services; Size Standard $5.0 million.
Scope of Work: (A) The contractor is required to provide: (1) Sorting
of all loose notes (medical and administrative) into terminal digit
order. (2) Drop filing of loose notes (medical and administrative) into
the appropriate patient record or outguide. (3) On-site supervisor and
appropriate personnel to accomplish task. The work must be completed
during other than the regular tour of duty, i.e., between 4:30pm and
8:00am, Monday through Friday and/or on the weekends. (4) Written
weekly updates identifying progress provided to File Room Supervisor
and Chief, Health Information Management. (5) Project completion within
three (3) months of start date. (6) A description of the method of
security to be utilized for maintaining privacy and confidentiality of
patient information. The contractor assumes responsibility for
compliance with the Federal Privacy Act in all aspects with regard to
material or data provided to the contractor relating to this project.
(B) Description of Work: (1) The current backlog consists of
approximately 90 feet or 108,000 documents. Documents include medical
and administrative notes. (2) All notes must be screened to identify
appropriateness for filing and sorted into terminal digit order. (3)
All notes must be drop filed into the appropriate patient record
(medical and/or administrative ) or into an outguide which is
appropriately filed. (C) Equipment: The VA Maryland Health Care System
will provide all equipment and supplies required, i.e., sorters,
carts, outguides, tables, chairs, and office supplies. The following
incorporated clauses and provisions are applicable to this acquisition:
52.212-1, Instructions to Offerors -- Commercial Items; 52.212-2,
Evaluation Commercial Items apply to this acquisition with evalution
criteria to include: (1) Technical Capability to meet agency's needs;
(2) Past performance; and (3) Price. (Award will be made to the offeror
obtaining the most points and whose proposal will be the most
advantageous to the Government. Award will NOT necessarily be made to
the lowest priced offer. All evaluation factors have equal weight.)
Offerors are to include a completed copy of the FAR provision 52.212-3
with their offer. Also included: 52.212-4, Contract Terms and
Conditions -- Commercial Items; 52.212-5, Contract Terms and Conditions
Required to Implement Statues or Executive Orders -- Commercial Items;
52.233-3, Protest After Award; 52.222-26, Equal Opportunity;
52.232-33, Mandatory Information for Electronics Funds Transfer
Payment. Contractors who wish to be considered must submit their
proposal on company letterhead along with commercial literature and
brochures on the service being offered that meets Government
requirements. Any interested parties must submit their proposals no
later than July 25, 1997 at 4pm, Eastern Standard time. No fax or
telephonic proposals will be accepted. The Government intends to
evaluate offers and award a contract without discussion with offerors.
Therefore, the offers should contain the offeror's best terms and
