Requirements for Managed Care Organizations. Training for Medical
professionals charged with the responsibility for implementing Clinical
Practice Parameters (CCP), Critical Pathways, and Disease Management
Programs for each Air Force Medical Treatment Facility. Course entails
the principles of disease management, the steps required for
implementation of any specific disease management effort, and disease
managements role in population health management. Contractor must have
experience in implementing programs themselves with out-come data. The
course must be taught in a 3-5 day block of instruction, impart how a
disease management program fit into population health, the information
systems, data collection, information feedback, patient
self-management techniques, the role of practice guidelines,
non-physician support personnel roles, collaborate care, with
observations extending into clinics where programs are in place for
observation. Course must encompass all diseases within a reasonable
