solicitation/synopsis for commercial items description, prepared in
accordance with the format in FAR 12.6, as supplemented with additional
information included in this notice.This notice constitutes the only
solicitation, a written solicitation will not be issued. The
solicitation, No. 263-97-P(BD)-0044, is issued as a RFP. The
solicitation/contract will include all applicable provisions and
clauses in effect through FAC 90-46. The standard industrial code is
8099, and the business size is 500 employees. The National Institutes
of Health, (NIA) Laboratory of Personality & Cognition, National
Institute of Aging (NIA) in Baltimore Maryland has a requirement to
acquire the services of a physician together with his or her intact and
current patient population at high risk for Human Immunodeficiency
Virus (HIV) infection. In particular, it is important to study the
behavior of some under-served populations of individuals, such as those
people within a minority group(s) withdifficult access. The NIH intends
to contract with Henry L. Masters, III, M.D. HealthSmart, PA., 425 W.
Capital Avenue, Suite 3812, Little Rock, Arkansas under the authority
of FAR 6-302-1 for these services It is the Government's belief that
Henry L. Masters, III, is the sole source for these services.
Administered to 190 participants (consisting of essentially minority
individuals) who reside in or represent urban and rural areas,
substance abusers, community recreation attendees, and correctional
inmates, who are also at high risk for HIV infection, the following
measures: 1) Slosson Oral Reading Test (SORT) 2) Personality assessment
using items from the Revised NEO Personality Inventory or the NEO
Five-Factor Inventory (if reading grade level on SORT less than seventh
grade) 3) Stages of Change for condom use for both main and casual
partners -- -five-item measure by Grimly, Prochaska, and Prochaska
(1993) -- (two separate four-item staging algorithms will be used, one
for condom use with a main partner and the second for condom use with
casual partners) 4) Self-Efficacy for Condom Use assessed with
five-item scale. 5)Ten-item scale of Advantages and Disadvantages of
Condom Use 6) Twenty Eight-item Processes of Change for Condom Use
scale. Outcome measures shall include: 1) Serologic test results for
HIV and syphilis 2) Quantitative HIV viral load and CD-4 counts.
3)Thirty six-item Medical Outcomes Survey (MOS SF-36). Score all tests;
enter data on a computer file (format to be determined by the Project
Officer); verify accuracy of data entry; conduct preliminary analysis
(e.g., frequency distributions); provide data analysis, data files, and
annotated code books to investigators at the Laboratory of Personality
and Cognition, Gerontology Research Center, National Institute of
Aging. FAR provision 52.212-1 Instructions to Offerors-Commercial,
applies to this acquisition. Award will be made to the technically
acceptable low offer. A completed copy of the provision 52-212-3,
Offeror Representatives and Certifications- Commercial Items, is
required as part of the offer. The clause 52.212-4 Contract Terms and
Conditions- Commercial Items applies to this acquisition.The clause at
52.212-5, Contract Terms and Conditions Required to Implement Statutes
Or Executive Orders-Commercial Items applies to this acquisition. The
Contractor will be paid on a partial payment basis. CBD NUMBERED NOTE:
Note 22 applies. Sources demonstrating the capability to accomplish
the above shall provide pertinent information that is in sufficient
detail to demonstrate their ability to perform. Information furnished
should include enough material to conduct a proper evaluation.
Respondents will not be notified of the evaluation results. Offer is
