synopsis/solicitation of a service contract to be obtained using
commercial practices in accordance with the format in FAR 12.6, as
supplemented with additional information included with this notice.
This announcement constitutes the only solicitation; quotes are being
requested and a formal solicitation will not be issued. This
solicitation, number F0863798QX024, is to be issued as a Request for
Quotation (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-46. This is a 100% small business set-aside, firm-fixed price
contract . The Standard Industrial Classification (SIC) Code is 8071,
with a business size standard of less than $5.0 million annually. (A).
Schedule: Contract Line Item Number (CLIN) 0001 -- The contract will
be for Computerized Axial Tomography (CT) SCAN Technician services for
the Base Year, 1 Oct 97 -- 30 Sep 98; CLIN 0002, Option Year One, 1
Oct 98 -- 30 Sep 99; and CLIN 0003 Option Year Two, 1 Oct 99 -- 30 Sep
00; CLIN 0004, Option Year Three, 1 Oct 00 -- 30 Sep 01; CLIN 0005,
Option Year Four, 1 Oct 01 -- 30 Sep 02; Pursuant to FAR 17.203(d),
bidders may offer varying prices for option periods. Services will be
provided Monday through Friday, 0730-1630 hours, (exclusive of
holidays), with one hour for lunch. Hours any less will be coordinated
with the Radiologist and the Quality Assurance Evaluator. (B)
Performance Work Statement: The work will include complete CT Scan
examinations in various areas, administrative duties associated with
CT, processing films and hanging for radiologic interpretations,
ordering of supplies, setting up protocol instruction in CT, and
administering training to military treatment facility (MTF) personnel.
Estimated procedures and estimated monthly data are as follows: CT
Chest -- 4, CT Chest with Contrast -- 5, CT Chest with/without contrast
-- 8, CT C-Spine -- 5, CT T Spine -- 1, CT L Spine -- 14, CT Abdomen
with contrast—15, CT Brain -- 12, CT Brain with contrast -- 13,
CT Sella -- 1, CT Orbins -- 2, CT Mastoid -- 1, CT IAM/IAC's 1 in 3
mos, CT Liver -- 3, CT Pancreas 1, CT Kidney -- 10, CT Extremities --
3, CT Pelvis with/without contrast -- 8. All examinations that can be
performed with existing equipment will be completed in the MTF. (C)
Place of Performance: 325th Medical Group (AETC), Tyndall AFB FL. (D)
Supplies/Services/Equipment/ Facilities: The MTF will provide: required
Government forms; all medical supplies commonly used in the facility;
full use of the Radiology Department and CT suite facilities;
Secretarial Services associated with CT work completed in the facility
and ancillary personnel required including medical assistants and
nurses; patient scheduling (complete administrative control of the
patient shall remain with the Government); and equipment as follows: GE
9800 Quick Computerized Axial Tomography Unit, Eastman Kodak Processor
(2 ea), Mass Film Viewer, and GE Dry Cam Laser Imager. (E) Method of
Performance: The contractor shall participate in Quality
Improvement/Risk Management activities as required by Air Force
Instruction (AFI) 44-119, and MTF plans and operating instructions. The
Government will evaluate the Contractor's performance under this
contract using quality assurance standards specified in AFI 44-119.
Inspections may also be conducted under the Inspection of Services
clause. The contract personnel shall comply with all safety procedures
and practices associated with the MTF and all Joint Commission for
Accreditation of Hospital Organization (JCAHO) Manuals during the term
of this contract. The quality of care shall meet or exceed standards
set by the JCAHO Accreditation Manual and the MTF Manuals. Personnel
Requirements: Services are to be performed by a CT technologist who is
CT registered by the American Registry of Radiologic Technologists
(ARRT), and holds a current state license. The technologist shall have
completed 2 years experience as a radiologic technologist with at
least one year full time employment as a CT technologist in a
multi-specialty facility. Experience must have been in performance of
unsupervised CT exams. These should include an estimated 200 yearly
each in the following major areas -- spine, sinuses, brain, chest,
abdomen, and pelvis; as well as 75 exams per annum of bone/joint
studies. Technologist must be able to work without supervision or
radiological assistance, except as needed to tailor the exam to the
radiologist's specifications. The MTF will initiate a Criminal History
Background Check on the CT Technologist within 15 days of start-date.
If the Contractor anticipates a temporary change in CT personnel,
he/she shall ensure the replacement is registered by the ARRT, has a
current state license, and has at least one year of experience in CT
Scan. No temporary replacement shall exceed 8 weeks for each period of
absence. The contractor shall provide the Contracting Officer copies
of the technologist's ARRT registry and experience at least three (3)
weeks before the change takes place. All credentials will be
reviewed/approved by the Chief of Radiology Services. No employee may
perform CT services under this contract unless he/she has passed a
physical examination within 30 calendar days before starting work. A
standard exam will include all items cited on Standard Form (SF) 88,
Report of Medical Examination, SF 93, Report of Medical History, and
observations and tests to ensure the employee is free of any contagious
disease. Examination shall also determine whether or not the individual
tests positive for Human Immunodeficiency Virus (HIV), or AIDS Related
Conditions (ARC) or Acquired Immune Deficiency Syndrome (AIDS).
Testing positive in any area of the examination will preclude an
employee from performing under this contract. Anyone who subsequently
develops any contagious disease or ARC/AIDS must immediately be removed
from contract performance, and the Contractor will provide a
replacement at no additional cost to the Government. At least five work
days before any employee performs CT services under this contract, the
Contractor shall provide certification that the employee passed the
required examination(s), to include date of examination, name and
address of physician performing exam, and a statement describing the
general health of the employee. The statement, consistent with
examination results, should be as follows: "(Name of the contractor
employee) is free from any contagious disease including, but not be
limited to, tuberculosis, hepatitis, and venereal disease; free from
ARC/AIDS, and has/has not (state which) tested positive for HIV."
Employees shall have a follow-up medical exam (as stated above, along
with the statement) within 30 calendar days of each yearly anniversary
of his/her contract performance. Additional examinations may be
required as necessary to protect the interests of the Government.
Personnel shall be neat, clean, well groomed, and dress in appropriate
hospital-type attire. Publications applicable during the life of this
contract include AFI 144-119, Quality Improvement in the Air Force
Medical Service; AFI 41-210, Patient Administration Functions; AFI
44-102, Patient Care and Management of Clinical Services; AFI 48-123,
Medical Examination and Standards; JCAHO Accreditation Manual; Medical
Group Instruction, Improving Organizational Performance Plan (IOPP);
and Diagnostic Imaging Services Operating Instructions. The contractor
will place its' personnel on an acceptable dosimeter program and
furnish them with dosimeters (radiation monitor), which must be worn by
the technologist. The MTF will provide emergency health care for
contractor personnel for injuries occurring while on duty. The
contractor will reimburse the Government for emergency health care
services based on $100.00 for an Acute Care Clinic visit. (I)
Clauses/Provisions: The provisions at FAR 52.212-1, "Instructions to
Offerors -- Commercial Items," applies to this service. The Government
intends to evaluate offers and award a contract without discussions.
Therefore, qualified contractors should submit a quotation for CLIN
0001, CLIN 0002, CLIN 0003, CLIN 0004, and CLIN 0005, each with the
best possible price. No partial bids will be accepted and the
Government may reject any or all offers if such actions are in the
public's best interest. Provisions at FAR 52.212-2,
"Evaluation-Commercial Items", apply to this solicitation and the
following evaluation criteria (arranged in their relative order of
importance) are hereby incorporated: lowest-priced, responsible
offeror, whose past performance is found to be acceptable, whose offer
conforms to the solicitation to satisfy the Government's needs, and
whose offer will be most advantageous to the Government. The clauses at
FAR 52.212-4 "Contract Terms and Conditions-Commercial Items," and
52.212-5 "Contract Terms and Conditions Required to Implement Statues
or Executive Orders- Commercial Items," apply to this acquisition [this
includes, but is not limited to, the following FAR clauses: 52.222-26,
Equal Opportunity; 52.222.35, Affirmative Action for Special Disabled
and Vietnam Era Veterans; 52.222-36 Affirmative Action for Handicapped
Workers; 52.222-37, Employment Reports on Special Disabled Veterans and
Veterans of the Vietnam Era]. The clauses at FAR 52.219-6, "Notice of
Total Small Business Set-Aside"; FAR 52.228-5, Insurance-Work on a
Government Installation; FAR 52.237.2, Protection of Government
