T -- PRINTING OF MEDICARE FEE SCHEDULE POC Carlos Morales, Senior Buyer (213) 741-6269. Transamerica Occidental Life Insurance Company (TOLIC) which currently is under contract with HCFA for Medicare Part B claims processing is seeking sources for the printing and distribution of the Medicare Fee Schedule. (I) TECHNICAL REQUIREMENTS: (1a) There will be approximately 4 different versions printed, (1b) Total quantity to print will be approximately 3O,000 copies, (2) Overall paper size 11 inches by 17 inches, folds to finished size of 8-1/2 inches by 11 inches, (3) Stock 16# white paper, (4) Text print in black ink, (5) Binding -- Saddle Stitch, (6) Approximately 15 double sided-pages, 11" X 17" which translates to 30 (thirty) 8 1/2" x 11" double sided pages, includes self cover -- front and back. (II) OTHER REQUIREMENTS: (1) Mail date is determined by HCFA, and once determined is not negotiable (approximately November 14, 1997), (2) Mailing- Bulk-rate Postage permit, (3) A 3480 cartridge or 3.5" diskette will be provided containing mailing list. (Please specify preference.) Vendor is responsible to generate labels or inkjet spray addresses on mailing, (4) Postage -- first class rate. Vendor is responsible for any prepayment, (5) Insert 1 additional page into approx. 4,000 booklets using wafer seal as self mailer, (6) Artwork will be available on 600 DPI laser print copy or on 3.5 diskette (Word 7.0) (Specify preference), (7) Capability to incorporate late text changes in one day, (8) Project must be completed within 6 to 10 days after receipt of artwork. All interested vendors must submit their responses to include the following: (1) Fulfillment cost to print and distribute approximately 3O,000 booklets, (2) Provide cost for up to 8 page decrease or increase of page count, (3) In-house fulfillment available, (4) Five client references, (5) Statement of previous experience, (6) SADBUS/MBE qualification statement, (7) Qualification statements stating vendor's ability to meet the Technical Requirements above (I-1a-6), (8) Responsiveness statement for meeting the Other Requirements above (II-1-8). Vendor bids will be evaluated 70% on cost and 30% on providing and meeting the requested information in technical and other requirements specified in I and II above.
