schurone@vafb5a.vafb.af.mil. The 30th Medical Group at Vandenberg Air
Force Base is soliciting for a Contractor to perform all laboratory
analyses, or refer specimens for analysis to a laboratory that
maintains current accreditation by the College of American Pathologists
(CAP) for the testing they perform. These laboratories must also comply
with provisions of the Clinical Laboratory Improvement Act (CLIA) of
1988. The Contractor will provide the 30th Medical Group Laboratory and
Medical Logistics with copies of CAP accreditation certificates.
Contractor will pick up specimens from the 30th Medical Group
Laboratory once each normal duty day of the 30th Medical Group, no
earlier than 1400 hours or later than 2000 hours for transport to
testing laboratory. Contractor will provide requisition forms for
ordering tests to the 30th Medical Group Laboratory. The Contractor
will provide containers necessary for transport that comply with all
applicable Federal, State and Local Laws for transport of biohazardous
material. The Contractor will provide collection material for
specimens that need to be collected in containers other than EDTA,
sodium fluoride, sodium citrate, or serum tubes. The Contractor must
provide two copies of their laboratory guide. The guide must include
information about the specimen required, patient preparation, reference
ranges and interpretive guidance. Contractor must provide updates or
revisions as they become available. Contractor will perform the request
assays. If unable to do so (missing information for the request,
improper or incomplete identification, wrong specimen), they must
notify the laboratory of the problem by telephone within one day of
discovery. Contractor will send 30th Medical Group Laboratory in
writing on their laboratory report form, the reason(s) for not
performing the requested test(s). Contractor will notify the 30th
Medical Group Laboratory of critical values as described by the College
of American Pathologists. Contractor will transmit results to the 30th
Medical Group, by courier orFAX, within one day of completion of
test(s). The period of performance for this acquisition will be 01
October 1997 through 30 September 1998. Award is contingent upon
availability of FY 98 funds This acquisition is subject to the
requirements of the Service Contract Act. DOL Wage Determination for
your locale will apply. This acquisition is set-aside for small
business. The standard industrial code if 8071, medical laboratories.
Size standard is $5.0 M. All responsible certified sources may submit
