Logistics Command has a requirement for a FUNDUS CAMERA WITH DIGITAL
IMAGING SYSTEM. This is a request for competitive proposals. This
requirement is a combined synopsis/solicitation for a commercial item
prepared in accordance with FAR 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only request for quotation: quotation will not be issued. Responses
must contain sufficient documentation to establish a bonafide
capability to fulfill the requirements. The request for quotation
document number is N62645-97-Q-1004. The request for quotation document
and purchase order incorporates provisions and clauses that are in
effect through Federal Acquisition Circular 90-46. The standard
industrial classification (SIC) code is 3841. The small business size
standard is 500 employees. Offeror shall provide complete system with
following essential characteristics: a. Unit shall consist of a table
top fundus camera and a 1024 x 1024 digitalimaging system. b. Fundus
camera shall be capable of stereo color photography, fluorescein
angiography, and Indocyanine Green angiography. c. Camera shall be
capable of viewing and capturing digital retinal images in 25+5
degrees, 40+5 degrees, and 55+5 degrees. d. Camera shall include a
manual tilting unit. Camera shall have at least a +30 degree lateral
swing and at least a +10 degree vertical swing. e. Camera shall be
capable of recording patient data on film. f. Camera shall include two
35mm motorized cameras, one Polaroid camera, and a motorized
instrument table. g. Camera shall include a fixation device. h. Unit
shall be capable of operating from 110+10% V, 60+2 Hz. i. Camera shall
have an image magnification on film of at least 1.7x at 55+5 degrees,
at least 2.5x at 40+5 degrees, and at least 3.4x at 25+5 degrees. j.
Camera shall have an image magnification of observation of at least
8.5x at 55+5 degrees, at least 12x at 40+5 degrees, and at least 17x at
25+5 degrees. k. Digital imaging system shall be capable of capturing,
manipulating, storing, and retrieving digital color fundus
photography, fluorescein angiograms and indocyanine green angiography.
l. Imaging system hardware shall include at dual CPU at least 200 MHz,
at least 4 GB Hard drive, at least 128 MB RAM, Jaz drive, at least 4x
CD writer, 21" SVGA monitor with minimum 1024 x 1024 resolution, at
least 33.6K modem, Ethernet card, SVGA video card with at least 4 MB
RAM, 1024 x 1024 imaging board, digital camera and connecting cables to
computer, additional high resolution monochrome monitor for real time
viewing of acquired image, system power center and backup, adapters
required to use with fundus camera, color laserjet printer, and system
desk. m. The printer shall have at least 300 pixels per inch
continuous tone, 64 MB onboard RAM, input tray, two SCSI ports, network
card for Ethernet, 200 8.5" x 11" color printer paper sheets, 2 color
printer ribbon with laminate patch. n. Unit shall include Windows based
software capable of acquiring, editing, storing, and compressing
images. o. STANDARD MANUALS. The offeror shall provide a minimum of two
copies of all standard manuals to include service (wiring diagrams,
schematics, and parts list), technical and operational manuals for each
component of system. p. Offeror shall deliver system to Tracy, CA. q.
Offeror shall be capable of providing service to Japan. The provision
52.212-1, Instructions to Offerors-Commercial Items, applies and offers
shall be evaluated for fair and reasonable price and technically
acceptability. Offerors shall include a completed copy of the provision
52.212-3, Offeror Representations and Certifications-Commercial Items,
with their offer. The following provisions and clauses apply:
52.212-1, 52.212-4, and 52.212-5. All responsible sources may submit a
