G -- CHEMICAL AWARENESS AND TREATMENT SERVICES FOR HOMELESS VETERANS SOL RFP 662-82-97 DUE 081897 POC Annie Bell, Contracting Officer, (415) 750-2240 The VA Medical Center, San Francisco, Ca., 94121 requires the lease of 20 beds for homeless veterans from a facility that is geographically accessible to the veterans, the VA Medical Center, San Francisco, and local bus transportation. Services shall also include lodging, on-site supervision, bed and bath linen service, 2-3 meals per day. Twenty beds shall be reserved for the Va Medical Center at all times (365 days). The VA shall provide individual case management and outpatient clinical services at its Medical center in San Francisco. The contract performance period is Aug 18, 1997 (or date of award), through July 31, 1998 with 4 one-year options through July 31, 2002. The facilities staff should have experience in mental illness, substance abuse and HIV disease and should be interested in helping this group and managing behavior. The VAMC intends to contract with the facility "A Man's Place, 399 Fremont Street, San Francisco, Ca., under the authority of FAR 6.302-1 for these services. CBD Numbered Note 22 applies. (0199)
