J -- FULL ON-SITE SERVICE, MAINTENANCE WITH PMI'S FOR AN ECHOCARDIOGRAPHY SYSTEM SOL IFB 521-014-98 (CG) DUE 090997 POC Christopher Grubbe, Contracting Officer, (205) 933-4492 The DVAMC, Birmingham, AL requires full on-site service maintenance, repair services and preventative maintenance for an Advanced Technology Laboratories, Echocardiography System, Model: Apogee CX200, for the period of 10-1-97 through 9-30-98, with four (4) one year option renewal options. The contractor will be required to perform preventative maintenance (PMI's) semi-annually. These PMI's shall be within the first five (5) working days of the months of October and April. All maintenance is to be in accordance with the manufacturer's instructions and must meet all NFPA and JCAHO requirements. Service calls will be five (5) days a week, Monday through Friday between the hours of 8:00 AM and 5:00 PM, including Federal Holidays. Service response time of four (4) hours from notification and on-site within eight (8) hours will be required. Specific reporting requirements will be required and will be strictly enforced.
