J -- FULL SERVICE MAINTENANCE AND REPAIR SERVICES FOR DIAYSIS SOL IFB 521-019-98 (EP) POC Edward G. Phillips (205) 933-4492 The DVAMC Birmingham, AL requires full maintenance and repair for 13 Fresenius USA, Inc Model 2008D and Model 2008E Diaysis Machines, for a period of 10-1-97 through 9-30-98, with four (1) one year renewal options. The Contractor shall perform quarterly preventive maintenance inspections (PMI) to be performed in October, January, April and July. These inspections are to be performed within the first five (5) working days of each of the months specified above. PMI's will be performed in accordance with applicable Fresenius 2008 Preventive Maintenance Manuals and shall meet all NFPA and JCAHO. Service calls will be five be five days per week, between the hours of 7:00AM and 7:00PM, Monday through Friday, Federal Holidays excluded. Contractor must respond to service calls within two (2) hours of notification and be on-site on the following business day. Specific reporting requirements will be required and will be strictly enforced. Request for bids sets must be in writing to the address listed above.
