J -- FULL MAINTANENCE AND REPAIR SERVICES FOR A WITT CORPORATION WITT SERIES PHYSIOLOGIC MONITORING AND WITT SERIES IMAGE ARCHIVAL SYSTEM SOL IFB 521-018-98 (WG) POC Wanda Goosby (205) 933-4492 The DVAMC Birmingham, AL requires full service sofeware/hardware maintenance and repair for a Witt Corporation Witt Series Physiologic Monitoring and Witt Series Image Archival System Model MS 0000, EE# 18377, for the period of 10-1-97 through 9-30-98 and four (4) one year renewal option years. The Contractor shall perform semi-annual preventive maintenance inspections (PMI) to be performed at six month intervals as follows: October and April. These inspections are to be performed within the first five (5) working days of each of the months specified above. PMI's shall be performed in accordance with manufactures's instructions and satisfy all requirements as set forth by NFPA and JCAHO. Contractor must respond to service calls within thirty (30) minutes of notification and be on-station within twenty-four (24) hours. Specific reporting requirements will be required and will be strictly enforced. Request for bids sets may be requested by fax at (205) 933-4489. No telephone request will be accepted.
