Proficiency in the following procedures are required: Screening of glaucoma, evaluation and prescribing corrective lenses, evaluation of patients with contact lenses to determine medical necessity, and diagnosis and treatment of other referred patients with conditions that would normally be treated by an Optometrist in the community. RFQ 8-0005 will be issued on or about July 30, 1997. Services will be required from the date of award or October 1, 1997, whichever is later for the base year, with four (4) one-year option periods.
