S. Army Medical Research Acquisition Activity (USAMRAA) intends to acquire Preventive Maintenance and Remedial Service on the Government-owned Meridian ACAS 570 Interactive Laser Cytometer at the Walter Reed Institute of Research (WRAIR), Washington, D.C. Contract performance period shall be 15 Sep 97 through 14 Sep 98 with 4-twelve month option periods. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for proposal (RFP) and the solicitation document and incorporated provisions and clauses are thus in effect through Federal Acquisition Circular 90-46. The contractor shall provide the following preventive and remedial maintenance services. 1. Preventive Maintenance (PM). The contractor shall provide two (2) PM inspections during the term of the contract. The first PM inspection conducted during the month March and the second during the month of September. The Government's Contracting Officer's Representative (COR) will be consulted by the contractor to establish a firm day and time the services will be scheduled. The inspections shall be performed independently from, and in addition to, remedial (repair) service that may be required during the term of the contract, unless the remedial service is performed within five (5) working days of a scheduled PM inspection. In such cases, the PM shall be accomplished simultaneously with the repair service and annotated as such. Each PM inspection shall provide the periodic service as specified by the equipment's manufacturer to maintain performance and shall include cleaning, lubrication, adjustment, calibration, and repair as necessary. NOTE: A DD Form 2163 (Medical Equipment Verification/Certification tag) furnished by the Government shall be filled out by the contractor servicerepresentative upon completion of preventive maintenance. 2. Remedial Service. The contractor shall respond to a call for remedial service within 48 hours after being notified of the need for such service. The contractor's service technician shall, within this time, inspect the equipment and determine the extent of repair(s) necessary to restore the equipment's proper operation. The repair(s) shall be completed within three (3) working days after the Government's notification, except when a repair part must be special ordered by the contractor, in which case the repair(s) shall be completed within four (4) working days after the Government's notification. NOTE: Remedial service calls shall be provided by the contractor on an unlimited basis during the term of the contract and at no additional cost to the Government. 3. Parts coverage. All parts that fail as a result of normal operation shall be replaced at the discretion of the service technician at no additional cost to the Government. All replacement partsshall be new or warranted by the contractor as equivalent to new. Excluded Parts: For purposes of this contract, all consumable type supplies used with the serviced equipment are specifically excluded from coverage under this contract, unless consumables are used on site by the contractor. When replacement of these parts and/or supplies is required to restore the equipment to operation, the Government will acquire such under a separate order. 4. Excluded Services. The contractor shall not be responsible under this contract to make repairs to equipment that has been misused or abused by Government personnel, damaged by a catastrophic event, or damaged in the course of relocating the equipment by Government personnel. 5. Conduct of Services. For equipment that is being serviced in Building 40, the contractor representative shall report to the Medical Equipment Maintenance Manager, Bldg. 40, Rm. B206A. Services under this contract shall be performed during the established working hours of the Government facility where the equipment is located, (Monday through Friday, 7:30 AM through 4:30 PM) unless otherwise indicated elsewhere in the schedule. During other than normal working hours, the contractor representative shall report to the CQ desk, main entrance, Bldg 40, excluding Federal holidays. a. Upon completion of service, a written services report shall be provided to the Medical Equipment Maintenance Manager. The report shall include detailed information regarding the cause of the equipment malfunction and corrective actions taken. Include the items to complete the work and a list of parts replaced with price of each part. b. In the event all information is not available to the contractor representative when services are performed, the initial services report shall include all available information listed above. The contractor shall provide the balance of the required information in writing to the Medical Maintenance Manager within ten days after the services are completed. The service report is to be forwarded or faxed to: Commander, WRAIR, ATTN: Medical Maintenance, Bldg 40, 14th & Dahlia St. NW., Washington, DC 20307-5100, Fax: (202)782-4648. 6. Contracting Officer's Representative (COR). The COR for all services covered under this contract is: SFC Curry (202)782-3696 or (POC) Mike Koenig (202)782-7517. All notifications to the contractor for remedial and preventive maintenance services will be made by the COR. The contractor's service technician shall contact the COR upon arrival at the Government activity before servicing the equipment. The COR shall be notified at least two (2) calendar days in advance of a scheduled PM inspection to insure contractor access to the equipment. After completion of repairs or inspection, the service technician shall provide a copy of a service report to the COR. 7. Travel and Transportation. All anticipated expenses for travel and transportation shall be included in the contract price(s) at the time of contract award. Any additional costs associated with travel and transportation over that which is included in the contract pricing must be borne by the contractor and will not be paid by the Government. 8. Warranty of Repairs. Equipment repaired and parts replaced by the contractor shall be warranted by the contractor against defects in workmanship and materials for a period of not less than thirty (30) calendar days following repairs or installation of replacement parts. 9. Liability of Damage. The contractor will not be liable for injury to Government personnel or damage to Government property arising from the use of equipment covered under this contract unless such injury or damage is due to the fault or negligence of the contractor. 10. Responsibility of the Government. Government personnel will not perform maintenance or attempt repairs on equipment covered by this contract unless agreed to by the contractor. If such action by the Government, which is agreed to by the contractor, results in continued malfunction of the equipment or damage to the equipment, the contractor shall remain responsible for ultimate repairs at no additional cost to the Government. Subject to the security restrictions of the Government activity where the equipment is located, the Government will permit the contractor access to the equipment to perform the necessary service. Contractor personnel shall submit to any reasonable Government-imposed security checks and/or identification. Proposal shall provide the per month unit price for Contract Line Item Numbers (CLINs): 0001 Provide maintenance services for Base period, 15 Sep 97-14 Sep 98 (Quantity-12 months); 0002 Provide maintenance services for Option year 1, 15 Sep 98-14 Sep 99 (Quantity-12 months); 0003 Provide maintenance services for Option year 2, 15 Sep 99-14 Sep 00 (Quantity-12 months), 0004 Provide maintenance services for Option year 3, 15 Sep 00-14 Sep 01 (Quantity-12 months); 0005 Provide maintenance services for Option year 4, 15 Sep 01-14 Sep 02 (Quantity-12 months). This solicitation is not set-aside for small businesses. FAR 52.212-1, Instructions to Offerors-Commercial Items (Oct 1995) is hereby incorporated by reference. Applicable is FAR 52.212-2, Evaluation-Commercial Items: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous, representing the best value to the Government price and other factors considered. Evaluation of offers shall be based on the following factors: (1) Technical capability of the services offered to meet the Government requirement, (2) Past Performance, and (3) Price. Factors are listed in their relative order of importance. Each offeror must (1) submit written information that pertains to their capability to perform the required services and proof of training completed by personnel which demonstrates knowledge of the ACAS 570 Interactive Laser Cytometer, (2) identify two federal, state, local government, or private contracts for which the offeror has performed work similar to the SOW in this solicitation (include contracts within the last two years, POCs, and telephone numbers), and (3) submit an offer. In addition, offerors are reminded to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items.
