Grubbe, Contracting Officer, (205) 933-4492 The DVAMC, Birmingham AL requires full on-site service maintenance and repair services and preventative maintenance for Forty Two (42) each Addressograph Farrington Card Embossers by Data Card, Incorporated. Model Numbers include #45, #860, #861, and #864. Period of coverage 10-1-97 through 9-30-98, with four (4) one year renewal options. The contractor will be required to perform quarterly preventative maintenance(PMI's) to be performed in three (3) month intervals. All maintenance is to be in accordance with manufacturer's instructions, and must meet all NFPA and JCAHO requirements. Service calls will be five (5) days per week, between the hours of 8:00 AM and 5:00 PM, Monday through Friday, including Federal Holidays. Service response time of four (4) hours from notification and be on-site within eight (8) hours will be required. Specific reporting instructions will be required and will be strictly enforced.
