synopsis/solicitation for commercial items prepared in accordance with
the subparts in Federal Acquisition Regulation 12.6, as supplemented
with additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and
a written solicitation will not be issued. Solicitation number 673-8-98
applies and is issued as a request for proposal. This solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 90-46. The Standard Industrial
Classification Code is 4119 and the business size standard is $5
million. Provide non-emergency transportation for stretcher care
beneficiaries of the James A. Haley Veterans Hospital, 13000 Bruce B.
Downs Blvd., Tampa, FL 33612. This proposal, if accepted shall be
effective for the period October 1, 1997 through September 30, 1998.
The Government may extend the term of this contract for two (2)
optional additional years, duration not to exceed September 30, 2000.
1. DELIVERY: Transportation services will be furnished on an "as
needed" basis. Coverage is required 24 hours per day, seven days per
week including holidays. Two (2) vans are to be made available for the
hours 7:00 a.m. to 7:00 p.m. Monday thru Friday and one (1) van at all
other times. 2. QUALIFICATIONS: a. Proposal will be considered only
from offerors who are regularly established in the business called for
and who financially responsible and have the necessary equipment and
personnel to furnish service in the volume required for all the items
under this contract. Successful offeror shall meet all requirements of
Federal, State, County, or City codes regarding operations of this
type of service. b. Each offeror must submit with his proposal a letter
in duplicate fully describing the number, make, and year of vehicles
which he agrees to furnish under this proposal including the location
and telephone number of his establishment where calls are received and
vehicles are immediately available for dispatch. Each offeror must
provide documentation that they meet all requirements of Federal,
State, County, or City codes regarding operation of this type of
service. 3. EQUIPMENT REQUIREMENTS: a. Vehicles shall be equipped as
follows: (1) Each vehicle shall have a crash stable side or center
mounting style litter fastener of the quick release type. (2) At least
two (2) strap type restraining devices shall be provided per
stretcher, cot and litter to prevent longitudinal or transverse
dislodgment of the patient during transit. (3) ALL VEHICLES WILL BE
AIR-CONDITIONED(4) Each vehicle shall have, in addition to the
rear-vision mirrors (required by the Morot Carrier Safety Regulations,)
an inside rear-vision mirror which will enable the driver to view the
entire passenger compartment, at the level at which the passengers
ride.(5) Vehicle entry and exit doors shall be equipped with latching
devices sufficient to retain individual passenger conveyances within
the passenger compartment of the vehicle regardless of the motion of
the vehicle. Striker plates will be used in conjunction with latching
devices.(6) Each vehicle must have a smooth floor which has a minimum
of voids or pockets at the floor to side wall areas where water or
moisture can become trapped.(7) The floor covering shall be of material
which can be maintained in a safe, sanitary, and odor free manner and
shall extend the full length and width of the patient compartment.(8)
Each vehicle shall carry, in a easily accessible location, a 1A:10B:C
fire extinguisher as a minimum.(9) The patient compartment shall
provide a minimum of 50 inches height measured from the finished floor
to the finished ceiling.(10) Each vehicle shall have an operable
two-way radio communications capability with the services base station.
b. Drivers shall be trained in the correct use of the special equipment
required herein, and shall insure that such equipment is in good
repair, and that each passenger is properly secured in place by
latching and restraining devices before the vehicle is placed in motion
and throughout the transportation performed. c. Sanitation and
Maintenance: (1) Equipment shall be cleaned routinely as needed.(2) All
lights must function properly.(3) All equipment in the patient area
must be adequately secured. 4 VEHICLE: The vehicle under the terms of
this contract will be licensed and meet the minimum vehicle
requirements as mandated by the state of Florida. 5 ATTENDANTS: a.
Contractor shall insure that each person employed is trained, qualified
and certified to perform the duties required. b. Record of each
employee as to character and physical capabilities of performing the
duties of a medical patient transport driver/attendant must be
maintained and made available for inspection upon request. 6 RESPONSE
TIME: In all cases patients shall be picked up within one hour of
requested pick-up time. 7 TREATMENT/MISTREATMENT OF PATIENTS IN ROUTE:
ANY SUBSTANTIATED MISTREATMENT OF PATIENTS IN THE PERFORMANCE OF THIS
CONTRACT WILL BE CAUSE FOR AUTOMATIC CANCELLATION WITHOUT A 30 DAY
NOTICE. DRIVERS MUST BE COURTEOUS AND CONSIDERATE OF ALL PATIENTS THEY
ARE TRANSPORTING. 8 ORDERS: a. Requests for services will be made by
telephone by the Contracting Officer or his designee. b. The Department
of Veterans Affairs maintains the right to use station vehicles prior
to using those of the contractor. c. If the contractor is unable to
furnish service within a reasonable time (1 hour) after receiving a
request or any order, the Department of Veterans Affairs reserves the
right to obtain the service from another source and to charge the
contractor with any excess cost which may result therefrom. The
Veterans Administration will be sole judge in determining when to order
service from another source. However, in no instance will the
contractor be required to furnish more than two (2) vehicles at one
time. Pricing shall be submitted for 1. Base Rate for pick-up or
delivery within 5 mile radius of James A. Haley Veterans Hospital, est.
25 ea/yr. 2. Base rate for pick-up ordelivery within 15 mile radius of
James A. Haley Veterans Hospital, est. 40/yr; 3. Base rate for pick-up
or delivery within 20 mile radius of James A. Haley Veterans Hospital,
est 180 ea/yr; 4. Base rate for pick-up or delivery within 30 mile
radius of James A. Haley Veterans Hospital, est 260 ea/yr; 5. Mileage
rate for pick-up or delivery for distance beyond the 30 mile radius of
James A. Veterans Hospital. Estimated 10 trips per month Rate per
mile. TOTAL AMOUNT FOR FY-98. Pricing shall also be submitted for
option year I (10/1/98 -- 9/30/99) and option year II (10/1/99 --
9/30/00). All proposals will be evaluated. The government intends to
make a single award to the responsible offeror whose offer is the most
advantageous to the government considering price and price related
factors. Provision 52.212-2 Evaluation Commercial Items, applies with
paragraph (a) completed as follows: "The following factors shall be
used to evaluate offers: (1) Past performance; (2) Qualifications
including but not limited to description of vehicles used; necessary
local, state and federal permits or licenses; staffing and (3) Price.
Technical and past performance, when combined are more important than
price. Offerors shall provide all necessary documentation needed
support technical evaluation of your offer. Offerors shall provide a
completed set of provisions with proposal found at 52.212-3, Offeror
Representations and Certifications -- Commercial Items. Provisions and
clauses found at 52.212-1, 52.212-2, 52.212-4, 52.212-5, 52.216-18,
52.216-19, 52.216-21, 52.217-8, 52.217-9, 52.232-19, 852.216-70, and
852.237-71 also apply. Proposals shall be submitted to the address
