SOL RFQ 552-2-98 DUE 082097 POC Contracting Officer, Kathy Laffey-Miller (937) 262-2138 This is a combined synopsis/solicitation for commerical items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotation, RFQ 552-2-98 is being requested and a written solicitation WILL NOT BE ISSUED. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAR) 90-46. This procurement is unrestricted. SIC code 7699 applies. Contract award is subject to the availability of funds. This will be a firm fixed price contract. Contract is to cover the base period October 1, 1997 thru September 30, 1998, with 3 option year renewals. Contractor shall provide all labor, parts deemed necessary for an unlimited number of on-demand service visits (excluding glassware for Dayton) and equipment to perform emergency repair and preventive maintenance on all BioMERIEUX VITEK equipment listed below. Service is to be provided 24 hrs. a day, seven days a week. There will be a 24hr response time. Item 1. Located in the MICROBIOLOGY LAB AT THE VA MEDICAL CENTER(VAMC), 3200 VINE ST., CINCINNATI, OH 45220. AUTOMICROBIC SYSTEM. Upon arrival at the station the Contractor shall contact Biomedical Engineering Room B4, 513-475-6420. Service calls shall be made by the Supervisor of Biomed or his designee. Item 2. Located in BUILDING 31 LAB, AT THE VAMC, 17273 S.R. 104, CHILLICOTHE, OH 45601. VITEK GJ060IMS. Services to be provide at Chillicothe includes, services performed at recurring intervals not to exceed 6 months and scheduling arrangements to make equipment available. Item 3. Located in the MICROBIOLOGY LAB AT THE VAMC 4100 W. 3RD. ST., DAYTON, OH 45428. ULTRAMICRO CLINICAL ANALYZER MODEL 2779. For all VAMC's the Contractor must provide a written description and frequency of the procedure to be performed on each equipment item. The description must be provided in sufficient detail to satisfy the standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). Upon completion of the work, the contractor must submit a legible copy of the service report to Engineering personnel. All work shall be performed by competent technicians, experienced and qualified to work on the equipment. All work performed shall be in accordance with commerical practices and the equipment manufacturer's specifications.
