a combined synopsis/request for quote for commercial items prepared in
accordance with the format in FAR subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only request for quote (RFQ); quotes are being
requested, however written RFQ's will not be issued. (ii) The RFQ
Number is F02601-98-Q0193. (iii) The RFQ and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-46. (iv) This RFQ is restricted to small business concerns. Large
business concerns may submit quotes with the understanding that their
quotes will only be considered if there are no quotes received from
small business concerns . SIC Code: 7699, Size Standard is
$5,000,000.00 (v) One line item. Maintenance of all equipment will be
considered one job and shall be billed on a quarterly basis. The
following is a list of the 13 pieces of equipment requiring preventive

Model No. 72-46HT. *Note: All equipment has been covered under an
existing maintenance agreement and is in good working condition.
Therefore, no formal site visit or inspection of equipment is required.
However, if you would liketo inspect the equipment, you may make
arrangements with the aforementioned point of contact. (vi) Contractor
must supply all labor, transportation, equipment, and materials
necessary to perform all scheduled preventive maintenance (PM) in
accordance with each manufacturers recommendation for periodic service.
If no guidance exists, PM services shall be accomplished quarterly and
as a minimum include cleaning, oiling, adjusting, and calibration. All
materials/parts which are required to be replaced during a PM shall be
included in the price of this contract. i.e. fluids, filters, and any
other parts which the manufacturer lists as requiring replacement
during an annual or quarterly PM service call. Additional parts which
require replacement, or parts required for "on-call" service, will be
billed at the same or lower price than that given to your most favored
customer. Parts and fluids removed by the contractor must be disposed
of properly at no additional cost to the government. Response time for
"On-call" requests for service shall be within 36 hours. To be
determined responsive, offerors shall provide a statement that they
have access to all manufacturers service literature and replacement
parts. Suggestions for improvements and efficiency of operation, should
be offered by the contractor to the customer at the time of service. A
detailed maintenance schedule must be provided with quote. All work is
to be accomplished on site at DRMO-UHB, Davis-Monthan AFB, Tucson,
Arizona. This is a firm fixed price request for quote. (vii) PM service
is to begin 1 Oct 97 and will expire 30 Sep 98. (viii) Federal
Acquisition Regulation (FAR) provision 52.212-1 Instructions to
Offeror-Commercial Items, applies to this acquisition. (ix) FAR
provision 52.212-2, Evaluation-Commercial Items, applies and quotes
shall be evaluated for fair and reasonable price, technical
acceptability, adherence to applicable clauses and provisions, delivery
schedule, and satisfactory performance record. (x) A completed copy of
the provision at 52.212-3, Offeror Representations and Certifications
-- Commercial Items must be submitted with quote (copies will be
provided upon request). (xi) 52-212-4, Contract Terms and Conditions --
Commercial Items applies. 52.212-5, Contract Terms and Conditions
Required to Implement Statutes or Executive Orders -- Commercial Items
applies. In accordance with the Service Contract Act, Wage
Determination No. 94-2025 Revision No. 8 Dated 11/25/1996 is hereby
incorporated into this solicitation. *Note: The following modification
has been issued to the aforementioned wage determination: reference
Fringe Benefits/Health and Welfare Rates, the hourly rate is changed
from $.90 to read $1.16 per hour. (xii) Quotes are due Not Later Than
26 Aug 97, 4:00 PM local time. Mail quotes to: 355th Contracting

85707-3522. Fax quotes are authorized and may be sent to (520)
228-5462. However, it is the offerors responsibility to ensure his
