Wolfe, 304/285-5883 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This notice, issued as Amendment 001 to Request for Quotation No. RQ97-025, answers questions by providing additional information and/or acceptable models for the items listed in the initial CBD Announcement appearing August 5, 1997. (001) Eppendorf Microcentifuge with rotor (#16-7012-03) or equal -- 3 each (lists part number); (002) Eppendorf Centifuge 5417R or equal -- 2 each (no change); (003) Orion PH Meter 611 or equal -- 3 each (no change); (004) Thermolyne Modular Heat Block, two place holder or equal 2 each (lists manufacturer); (005) Heating Block 24x1, 5ML, Micro 2 each; (006) Modular Heating Block, 12-16mm 2 each; (007) Modular Heating Block 20-30mm 2 each; (008) Analytical Topload Balance 210G with screen, 4 place 1 each (defines specification, no preference); and (009) Precision Topload Balance, 500G with screen, 3 place 1 each (defines specification, no preference). The Government will evaluate based on F.O.B. Destination (Morgantown, West Virginia) and the requirements at FAR 52.214.21 "Descriptive Literature" apply to referenced brand name products. Information provided must demonstrate products offered are equal or your offer may be rejected. The Government desires delivery of all units within 2 weeks after award of order. Offers shall state a delivery schedule. All FAR provisions and clauses stated in the initial notice apply without change. The due date for quotes is extended to August 29, 1997, 2:00 p.m. local prevailing time. Submit the following to DHHS, PHS, CDC, NIOSH, ALOSH, Attn: Brenda Goodwin, 1095 Willowdale Road, Morgantown, WV 26505: (1) original and two copies of a completed SF 1449, signed by an individual authorized to bind the organization; (2) Schedule of Offered Supplies/Services containing pricing of individual items (001-009) being offered, delivery schedule and published price lists (if applicable); (3) completed Representations and Certifications -- Commercial Items, FAR 52.212- 3; (4) descriptive literature, tables, etc. which demonstrate compliance of equal products for evaluation purposes; and (5) acknowledgment of solicitation amendments, if any. Offers that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration.
