provide all labor, equipment, and materials, to perform duties as a
Health/Fitness Instructor, to include but not limited to: DUTIES AND
RESPONSIBILITIES (1) Conduct various aerobic and specialized exercise
classes. Instruct fitness training programs, and fitness assessments.
(2) Coordinate with the exercise physiologist on Cycle Ergometry,
assessments and training of active duty members for determination of
VO2 Max. May be required on occasion to administer the Cycle Ergometry
assessment to individuals. (3) Check fitness equipment on a routine
basis and notifies the manager of Health Promotions Program of
equipment breakage or malfunction. ensures that participants do not
misuse the equipment, prevents access to defective equipment, and
ensures safety of all participants. (4) Present fitness equipment
orientation and education to participants, which include topics such as
safety, proper warm-up, proper stretching, explanation of physical
fitness cardiovascular training, strength training, flexibility,
correct exercise techniques and basic nutrition. (5) Coordinate with
the installation fitness program manager, and performs fitness
assessments and prescribing exercise prescriptions. (6) Coordinate with
the installation fitness program manager and develop health programs
for the local population as determined by a local needs assessment
survey. (7) Submits the following reports for review and acceptance by
the Contracting Officer or an appointed representative: a. Schedule or
aerobic/exercise classes and number of participants monthly. b. An
outline for participants on topics such as proper warm-up, proper
stretching, correct exercise techniques etc. c. Documentation on
fitness assessment on participants and number of participants assessed.
d. Log of equipment maintenance and cleaning. REQUIREMENTS FOR THE
POSITION: (1) Knowledge: Contractor shall have knowledge of physical
fitness and exercise, BS degree in the health fitness field preferably
a masters in exercise physiology. Must havecurrent certification as a
Health/Fitness instructor with the American College of Sports Medicine
and the American Council of Exercise (ACE) or American Fitness Aerobic
Association (AFAA), and shall have current CPR certification. (2)
Experience: Experience of at least one year of practice which enables
the employee to competently and independently lead aerobic/fitness
classes, perform fitness assessments. Ongoing experience in working
harmoniously with groups of people within a health and wellness
setting. Experience with submaximal assessments and the AF Fitness
Program helpful but not required. (3) Duties and Responsibilities:
Prescribe exercise prescription and direction in the use of exercise
and equipment, i.e. treadmills, stationary bikes, Cybex strength
equipment. Performs fitness assessments to include cardiovascular
endurance, flexibility, muscular strength and endurance and skin-fold
caliper measurement for body fat, and bio-impendence body composition
analysis. Aids in providing programs to meet the needs of a large
number of participants. (4) Complexity: The work includes various
duties involving participation in the exercise programs, and assisting
in the implementation of individual programs. The Fitness Instructor
schedules fitness assessment appointments and reevaluations. Works
directly with the serviced population and with all levels of
installation management to help promote and conduct health and fitness
programs, aerobic programs, HAWC educational programs, etc. (5)
Physical Demands: The contractor is required to do considerable walking
and bending, stooping, stretching, demonstrating physical flexibility.
May be required to drive a vehicle when participating in an off-site
Health and Wellness program. Much of the work of this position is done
in a Health and Wellness Center setting. Moderately heavy physical
effort may be required while leading exercise classes. May be required
to lift objects weighting up to 50 pounds. GUIDELINES: Guidelines
include established written procedures of AFI 40-101, AFI 40-501, AF
PGL 94-8 and Nellis HAWC OI's. Deviations from guidelines are
coordinated prior to implementation. POSITION OVERSIGHT: Objectives and
priorities of the program are reviewed and accepted by the Director of
Health Promotion Programs who coordinates overall objectives and
priorities of the program. The base installation base fitness program
manager is the expert in the technical aspects of exercise programs and
in fitness activities. Work is reviewed for consistency with broad
program objectives and compliance. PERSONAL CONTACTS: Since work
requires contact with people from all walks of life the fitness
instructor must have an ability to work independently and at ease with:
all ranks of the military, active duty and retired and their
dependents, base civilian employees, other staff members of the HAWC,
students, professionals in the civilian community, individual
participants, and the base medical facility. PURPOSE OF CONTACTS:
Contacts are to exchange information concerning individual goals
regarding their physical and emotional fitness, to instruct fitness and
wellness, and to monitor or adjust individual fitness programs. PLACE
OF PERFORMANCE: Work will be performed in the exercise/fitness area
within the Health and Wellness center, building 625, except as
otherwise specified by the Director of the Health Promotion program.
Advance notice of 2 working days will be given. Offerors will bid on
the following items: CLIN 0001, Health/Fitness Instructor, Quantity 12,
Unit Months, Unit Price, Total Price. This is a combined
synopsis/solicitation for commercial services prepared in accordance
with the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This solicitation (F26600-97-QV129) is
issued as a Request for Quotation. This procurement is reserved for
small business concerns, utilizing the size standard in FAR 19 under
Standard Industrial Code 8049 This solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 90-32. The provision at FAR 52.212-1, Instructions
to Offerors-Commercial applies to this solicitation. The clause at FAR
52.212-4 Contract Terms and Conditions-Commercial Items applies to
