804-675-5490 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. Solicitation
No. RFQ 652-8-98. This solicitation document and incorporated
provisions and clauses are those in effect through FAC 90-46 (#57) and
VAAR 96-28 (#14). SIC Code 7218 applies. Size Standard is $7.0
million. This procurement 100% set-aside for small businesses. The
Hunter Holmes McGuire VA Medical Center, Richmond, Virginia is seeking
a contractor that can provide dry cleaning services for Fort Lee
Uniforms in accordance with the specifications and requirements of this
solicitation. The contract period will cover the period October 1, 1997
through September 30, 1998 with options to extend for FY 99, FY2000,
FY2001, and FY2002 (4 option years). Award of a Firm Fixed-Price
Contract will be made for the base year period plus the four option
years. Scope of Work: (a) The contractor will provide all equipment,
labor, and materials to dry-clean uniforms in accordance with industry
standards. (b) All uniforms will be cleaned, pressed, and placed on a
hanger. A piece of plastic will be placed over each uniform. Uniforms
will be maintained separately and tracked to avoid loss. (c) Any
damage or loss to uniform will be repaired or replaced by the
contractor at their expense. (d) All uniforms must be completed within
two (2) working days to be picked up. (e) The contractor will maintain
accurate accounting records for billing customers monthly. (f) The
contractor will dry-clean curtains or special requested items in a
timely manner. Supervision By Contractor: The contractor shall appoint
a project manager who will have responsibility for performance of
equipment repairs. The project manager shall be competent to supervise
all aspects of services and shall be available on-site within two (2)
hours after notification (Monday through Friday, 8:00 a.m. through
5:30 p.m.) when problems arise with contractor's work performance or in
the event emergencies arise at this facility which pertain to
contractor's responsibilities or duties. The project manager shall, in
the absence of the contractor, act for the contractor as contractor's
representative. Quality Control/Quality Assurance: The government will
monitor the contractor's performance monthly using a Quality Assurance
Work Performance Form (to be provided to contractor awarded the
contract). Quality Assurance Work Performance Review Form will consist
of the following elements and evaluated accordingly: (1) Cleanliness
of Uniforms (a) Whiteness (b) No Stains (c) Free of Other Material
(Rated: Poor/Fair/Good/Excellent); (2) Appearance (a) Pressed Neatly
(b) No Wrinkles (c) Customer Sorting/Hanging Items Together (Rated:
Poor/Fair/Good/Excellent); (3) Timeliness (a) Processed within two
working days (b) No losses (c) Accurate Billing (Rated:
Poor/Fair/Good/Excellent). A copy of monthly evaluations will be
provided to the VA Contracting Department on a monthly basis.
Contractor to obtain any requested copies through the VA Contracting
Department. EMS will maintain a master file of all evaluations within
their main office area. The following provisions apply to this
