Lancaster Rd., Dallas, TX 75216 intends to award a contract for a CE-2000 Cryosurgical System (2 each), #507-1 Mobile equipment carrier (2 each), #171 18 gauge cone probe (4 each), #139 Standard spherical retinal probe (4 each), #134 20 gauge vitreous probe (4 each) and #163 Fast freeze probe (4each) with Cooper Surgical, 15 Forest parkway, Shelton, CT 06484. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a request for proposal.
