Z -- CARPET REMOVAL AND REPLACEMENT, BLDG 1805, CHILD DEVELOPMENT CENTER SOL F11623-97-T3794 DUE 090497 POC SSgt Scot Gagnon, (618) 256-1500, ext 436; Dennis Harmon, (618) 256-1500 ext 435 (i) This is a combined synopsis request for quote for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcment constitutes the only request for quote (RFQ); quotes are being requested and a written RFQ will not be issued. (ii) The RFQ Number is F11623-97-T3794. (iii) The RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. (iv) This RFQ is 100% set-aside for small business concerns. SIC Code: 1752, Size Standard is $7.0M. (v) One line item. Carpet Removal and Replacement in Bldg 1805 is considered one job, lump sum. (vi) Services, non-personal: Contractor to provide all labor, materials, equipment, transportation, and other items and services necessary to remove and replace carpet throughout Bldg. 1805, Child Development Center, Scott AFB IL. The Contractor shall remove and dispose, off-base existing glued down carpet in corridors, offices, and classrooms. Floors shall be properly prepared for the installation of new carpet, to include scraping of excess old glue, carpet backing residue, etc. The contractor shall purchase and install an estimated 1100 SY of carpet in corridors, offices, and classrooms. Carpet shall conform to the following Brand Name specifications OR be of equal specifications: Manufacturer: Patcraft Commercial Carpets, Style: Paradox, Number: 18440, Color: 215 Analysis, Yarn Type: 100% Premise Bulk Continuous Filament Nylon. Pile Construction: Loop Pile, Dye Method: 85% Solution Dyed, 15% Space Dyed, Gauge: 1/10 Stitches Per Inch: 11.5, Tufted Pile Height: 5/32", Tufted Yarn Weight: 28 oz., Density: 7149, Special Treatments: Fluorochemical & The Inhibitor Anti-Microbial Treatment, Backing: Enhancer III, flooring Radiant Panel: Class I (Direct Glue Method), Traffic Class: Extra Heavy. The Contractor shall submit a sample of the Brand Name or equal carpet, along with the carpet specifications, at the time the quote issubmitted. (Interested parties may request a copy of the floor plan, identifying the carpeted areas, by e-mailing gagnons@wing.safb.af.mil or faxing a request to 375 CONS/LGCV, Attn: SSgt Scot Gagnon, (618) 256-4325. Requests should include company name RFQ number, company phone number, address, and fax number.) The Contractor shall provice nontoxic adhesive for caprpet that adheres to the manufactures specifications for this type of installation. All seams and joints shall be properly sealed. The contractor shall purchase and install new vinyl 4" base cove throught the building. Base cove shall conform to the following brand name specifications: Brand: Johnsonite, Color: CB-45 Sandalwood. The contractor shall submit a sample of the brand name or equal base cove, along with base cove specifications, at the time the quote is submitted. The contractor shall provide the vinyl base nontoxic adhesive that is appropiate for gluing base cove to walls. The contractor shall be responsible for removal and disposal, off-base, of all cuttings and scrap pieces from the work site. The contractir shall leave the area ready for occupancy. The contractor shall remove and replace all door stoppers and door guards that are in the way of carpet installation. The contractor shall remove and replace rubber stripping that divides the tile and carpet in the classrooms and doorways. The contracitor shall be responsible for the removal of all furniture that is presently on the carpeted areas and return the furniture to the same places so the room(s) will be ready for occupancy. The contractor will unbolt items that are secured to the wall and/or each other and securely rebolt them back to origional location. This is a firm-fixed price request for quote. (vii) All work shall be completed when the building is closed to children; 6:00 p.m. until occupancy), and anytime on Saturday and Sunday (rooms shall be completely put back together by 6:00 a.m. Monday for daytime occupancy). Work shall commence on or before 27 Sep 97. The contractor shall contact the designated Government Point of Contact at least 7 calendar days prior to starting work to coordinate scheduling and access to the work site. The contractor shall specify the number of days required to complete the above outlined work in the submitted quote. (viii) Federal Acquisition Regulation (FAR) provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) Far provision 52.212-2, Evaluation-Commercial Items, applies and quotes shall be evaluated for fair and reasonable price, technical acceptability, adherence to applicable clauses and provisions, and satisfactory performance record. (x) A completed copy of the provision at 52.212-3, OfferorRepresentations and Certifications-Commercial Items must be submitted with quote (copies will be provided upon request). (xi) FAR provision 52.212-4, Contract Terms and Conditions-Commercial Items applies. (xii) FAR provision 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies. (xiii) In accordance with the Service Contract Act, Wage Determination No. 94-2309 Revision No. 6 dated 12/12/95 is hereby incorporated into this solicitation. *Note: the following modificationn has been issued to the aforementioned wage derermination: reference Fringe Benefits/Health and Welfare Rates, the hourly rate is changed from $.90 to read $1.16 per hour.
