65 -- DIGITAL COMBINATION EEG/SLEEP ACQUISITION, REVIEW AND ANALYSIS COMPUTERIZED SYSTEM SOL RFQ 546-13-97 DUE 090497 POC Jane Ferencz 305/324-3107 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplimented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. This solicitation no. RFQ 546-13-97 is issued as a Request for Quotations. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 46. The Standard Industrial Code (SIC) is 3845 and the small business size standard is 500 employees. Contract will be firm, fixed-price. Requirement is to provide FOB Destination to the VA Medical Center, Miami, Florida, a digital combination EEG/Sleep acquisition, review and analysis computerized system to include EEG collection and analysis package, video acquisition and analysis package, sleepscan collection package, sleep extended analysis, with license, on-site installation and user training, and a one year warranty to include all parts and labor. System must have the ability to download existing data and interchange equipment components with an existing Ceegraph SE System. Ceegraph SE System by Biologic Systems or equal. Pursuant to 52.212-2 the Government intends to make a single award to the responsible offeror whose offer is the most advantageous to the Government. Award will be based on (1) technical acceptability as determined by the product literature submitted which demonstrates ability to perform required functions, (2) past performance and (3) price.
