(513)475-6339 RFQ#539-98-01 is a combined synposis/soliciation for
commercial items in accordance with the format in FAR Subpart 12.6, as
supplemented with information included in this notice. This
announcement constitutes the only solicitation. Quotes are being
requested, and a written solicitation will not be issued. The
contractor shall pick up, fill and deliver GOVERNMENT-OWNED cylinders
of gaseous oxygen, nitrogen and nitrous oxide. The gases are needed at
the VA Medical Center in Cincinnati, Ohio and the VA Ft. Thomas
Nursing Home in Ft. Thomas, Kentucky. The SIC code is 2833, and the
small size standard is 1,000 employees. This is an unrestricted Request
for Quotation. Bidders shall provide a price schedule for a base year,
October 1, 1997 thru September 30, 1998, with 4 renewable option
years. Prices quoted are to be quoted PER CYLINDER. Prices are to be
FOB destination and any applicable hazardous material charges should be
included in the cost per cylinder. The quantities listed are estimates
of each division's annual usage. The cylinder sizes are approximate,
but they generally follow the industry standard. This solicitation is
for a firm fixed price requirements contract. REQUIREMENTS: Pick ups
and deliveries of the following gases are to be made to the VA Medical
Center ATTN: Warehouse-Cylinder Room, 3200 Vine Street, Cincinnati, OH
45220 between 8AM and 4PM. The requirements are: 1) Oxygen, gaseous "E"
cylinder, approximate capacity is 25 cubic feet, 1500 cylinders per
year, 2) Oxygen, gaseous "H" cylinder, approximate capacity is 220
cubic feet, 5 cylinders per year, 3) Nitrogen, gaseous "E" cylinder,
approximate capacity is 25 cubic feet, 5 cylinders per year, 4)
Nitrogen, gaseous "H" cylinder, approximate capacity is 255 feet, 70
cylinders per year, 5) Nitrous oxide "E" cylinder, approximate capacity
is 25 lbs., 7 cylinders per year, and 6) Nitrous oxide "H" cylinder,
approximate capacity is 64 lbs., 25 cylinders per year. The cylinders
are generally picked up on Tuesdays, filled and returned on Thursdays
(except on Federal Holidays). Pick ups and deliveries to the VA Ft.
Thomas Nursing Home are to be made to the South end of the building
loading area at 1000 South Ft. Thomas Avenue, Ft. Thomas, KY 41075. The
contractor will be will be provided a key. The requirements are: 1)
Oxygen, gaseous "E" cylinder, approximate capacity is 25 cubic 250
cylinders per year, and 2) Oxygen, gaseous "H" cylinder approximate
capacity is 220 cubic feet, 1400 cylinders per The cylinders are
generally picked up on Mondays, filled and delivered on Thursdays
(except on Federal Holidays). PLEASE NOTE THE DELIVERY REQUIREMENTS FOR
BOTH DIVISIONS -- A WEEK TURNAROUND TIME BETWEEN PICKUP AND DELIVERY IS
UNACCEPATBLE. The contractor shall be responsible for notifying the
ordering activity prior to disposal of non-repairable government-owned
cylinders, parts etc. Disposition shall be made according to the
ordering activity's instrutions. The contractor shall be paid for all
services peroformed and authorized by the ordering activity, up to the
determination that a cylinder is not repairable. The contractor shall
also be responsible for notifying the ordering activity of any
cylinders requiring hydrostatic testing. ADDITIONAL ITEMS that may be
required. The bidder shall also include pricing for the following
services/parts that mya be required during the contract period. The
pricing of these items will be considered but not as a major pricing
factor. The services are: one color painting of cylinder, stencilling
cylinder, hydrostatic testing (includes testing, indenting, new test
date, drying interior and resetting valve), modified hydrostatic
testing (when permitted), dehydrating cylinders (when not part of
hydrostatic testing), internal cleaning cylinders (includes pruging,
cleaning and drying), cleaning contaminated cylinders as required
includes pressure test evacuation, purging, pressurizing and analysis
to assure contamination is removed), removal of fractured, rusted or
damaged caps, valves or plugs, and stamping valve with the name of the
gas. PARTS that may be required are: valve, dip (siphon tube), collar,
valve hood, safety device cap, plastic outlet cap or plug, outlet cap
or plug with chain and dip, frangible disc, fusible plug in valve,
fusible plug in cylinder, valve stem and valve packing. NOTE: Any of
the parts/services will be issued on separate credit card orders.
BIDDERS MUST ALSO INCLUDE A COMPLETED COPY OF THE PROVISION AT
52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS -- COMMERICAL
ITEMS, 52.204-6 CONTRACTOR IDENTIFICATION NUMBER -- DATA UNIVERSAL
NUMBERING SYSTEM (DUNS) NUMBER AND VAAR 852.219-70, VETERAN OWNED SMALL
BUSINESS WITH ITS QUOTE. The following incorporated provisions and
