This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. Solicitation DADA16-97-T-0039 herein is a Request for Quotation (RFQ). The solicitation document, provisions and clauses herein are those in effect through FAC 90-46. The proposed solicitation contained herein is restricted to Small Business Concerns. Delivery of the procured supplies shall be FOB DESTINATION, Tripler Army Medical Center, Hawaii. The Government anticipates to award a Firm Fixed- Price contract for the period of 1 Oct 97 -- 30 Sept 98. Line item 0001: EG7+ cartridges (12000 ea), Delivery Schedule for line item 0001 will be 1000 per month beginning 15 Oct 97, with a delivery every 15th of the month thereafter up to 30 Sept 98. Line item 0002: G3+ cartridges for (7800 ea), Delivery Schedule for line item 0002 will be 650 per month beginning 15 Oct 97, with a delivery every 15th of the month thereafter up to 30 Sept 98.These cartridges will be used on Contractor leased equipment from I-STAT CORP, model number: P-96314, serial numbers: 2-25448, 2-24801, 2-26045, 2-25041, 2-21244, 2-24995, 2-25446, 2-24132, 2-25043, 2-24134, 2-25447, 2-24139. The following factors will be used to evaluate offers in descending order of importance, (1)price, (2) technical capability of the item offered to meet the government's requirements, (3) past performance. Award will be based on the lowest priced, technically acceptable offer from a vendor with a satisfactory past performance record. The offeror contractor shall provide a list of at least 3 contracts awarded to your firm for similar and identical supplies with government and commercial concerns. Your ability in the area of workmanship quality, performance, efficiency, responsibility, and timeliness, and reasonable and cooperative behavior and commitment to customer satisfaction, and complaints will be evaluated.
