(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation, F11623-97-T3097, is issued as an request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 90-46. (iv) This is a total small business set-aside. The associated standard industrial classification code is 4953 and small business size standard $6.0 million. (v) There is one line item number and quantity is 12 months (MOs) (vi) Description of requirements for the items to be acquired. Services, Non-Personal: Contractor shall provide all labor, tools, equipment, materials and transportation required to land apply 400,000 gallons of liquid sludge and 1200 cubic yardsof cake sludge from the Scott AFB Waste Treatment plant. Contractor will furnish all sludge application documentation to waste plant foremen by 20 days after application. Contractor must be fully accredited to apply waste sludge and comply with all United States and Illinois EPA rules and regulations including CFR40-503. (vii) Period of performance is 1 Oct 97 through 30 Sep 97. (viii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The FAR provision 52.212-2, Evaluation -- Commercial Items, applied and quotes shall be evaluated for fair and reasonable price, technical acceptability, adherence to applicable clauses and provisions, and satisfactory performance record. (x) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, must be submitted with quote (copies will be provided upon request). (xi) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
