SOL RFP 609-05-98 DUE 110397 POC Mary Bird, Contracting Officer (618)993-4175 The VA Medical Center, Marion, Il is seeking offers from medical care providers for primary health care services for enrolled veterans. Payment of services will be on a monthly capitated rate basis. The contract term will be one base year with two 1 year options for contract extension. The primary health care services will be provided in Carmi, IL (White Co.). The facility must be in compliance with the American Disabilities Act requirements. The VA Medical Center is expecting to enroll approximately 1500 -- 2000 patients one year after award. The contractor will provide all personnel, clinic facilities, equipment, supplies, and supervision for the provision of primary health care services. The contractor will be responsible for the provision of all radiological and laboratory services associated with primary health care covered by the contract. The contractor will be responsible for providing pharmacy prescriptions using VA formulary. The contractor must have a primary care provider on call 24 hours per day, 365 days per year to respond to enrolled patient telephone inquiries. Contract award will be based on best value to the VAMC using evaluation of offerors as determined by selection factors, including cost as one selection factor. Solicitation documents are expected to be available on or about October 1, 1997. Offer due date is expected to be on or about November 3, 1997.
