This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in FAR 12.6, the test program in FAR
Subpart 13.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation will not be
issued. (ii) This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 90-45.
(iii) Walter Reed Army Medical Center has a requirement for a 32
Channel Cardiolab Integrated Electrophysiology System. (iv) GENERAL
INFORMATION: 32 Channel Cardiolab Integrated Electrophysiology (EP)
System minimum essential characteristics are as follows: A system which
integrates the annotation and storage of Fluoroscopy Image data with
the patient signal data. Providing real time display of live
fluoroscopy image. Isochronal and isopotential mapping capabilities to
the EP system and provides integrated benefits in conjunction with the
image system. A RF Ablation interface module which shall be able to
display and integrate into the system log, the calibrated digital data
output by supported generators. Instantaneous 12 lead printouts and
real time signal trace recordings. 1600 X 1280 pixels video displays
with correlated data. Video system shall be able to provide continuous
patient monitoring and during reviewing already acquired data not
interrupt real time video display. Computer System shall be at a
minimum 180 MHZ Pentium with 32 MB of memory and 1.0 GB hard disk with
33.3 baud modem. The system shall be multitasking so that any
combination of recording and analysis functions may be performed
simultaneously. The computer shall have three independent 32 bit high
speed microprocessors to provide a real-time multitasking, highly
responsive data acquisition and analysis system. Configuration shall be
able to be user-defined, and the system can contain a virtually
unlimited number of configurations. Computer shall have an interactive
log providing instant, interactive access to all of the data from the
study. The log shall be continuously visible on the screen, regardless
of the other functions that are in use at that time. Computer
capabilities shall make it possible to change most signal parameters,
including gain, clip, and sweep speed by utilizing a simple keyboard
operation. Entries in the log shall be made by pressing a single
user-defined function key. The system shall contain a complete patient
catalog, maintaining a database of all optical disks used and the
patients on each disk. The catalog shall be searched and printed in a
variety of formats. The system shall be protocol-driven, allowing the
system to automatically change many parameters such as screen display,
stimulus input, stimulus sync channel, log, and measurement formats by
simply changing the current protocol. (v) QUANTITIES AND PRICING:
Pricing shall be provided for the following quantities of each
component of the system. The pricing shall include costs for equipment,
software, training, installation and documentation as it applies to the
system. Provide quantity discounts when applicable. (vi) ITEMS:
Contractor shall provide Technical Descriptive Literature in sufficient
detail to demonstrate compliance with this requirement. a) One (1) each
32 Channel Cardiolab Integrated Electrophysiology System. b) One (1)
each Amplifier Unit. c) Two (2) each 21" flat monitor screens, high
resolution, (1600 X 1280) color monitors. d) Two (2) each Ultra High
Resolution Video Boards, (1600 X 1280 pixels). e) LaserJet printer with
600dpi capability, 12 page/min and full page graphics with 2 MB of
memory. f) Software -- Current version of Cardiolab software, Modem
communication for instant support, Built-in fax capability. g)
Accessories: Workstation mobile cart with swivel arms, 60" W X 34" H X
30" D (153cm W X 86cm H X 77cm D), 33.3K Baud modem, Spare 50' optical
cable, Spare set (2) catheter input modules, Four (4) 600 MB optical
disk cartridges, Isolation safety transformers. h) One (1) each
CardioImage (TM) Fluoroscopy Image Management System. i) One (1) each
CardioMapp Integrated Mapping Software Module. j) One (1) each External
Device Interface Module. k) One (1) each Four Channel High Level A/D
Board. l) One (1) each Pressure Transducer Adapter for CardioLab
Amplifier. m) One (1) each Limb Leads, Translucent, D Series Comp, 36",
Set of 5. n) One (1) each 21" Flat Screen High Resolution Auxiliary
Monitor (1600 X 1280). o) One (1) each Cable, video extension cable
RGBHV BNC connectors, 50'. p) One (1) each Strip Chart Recorder-200,
resolution of 200 dpi, paper speed up to 200mm/sec., Print up to 32
channels from screen display, Instant 12 lead printout, Dimensions:
13.25" W X 6" H X 17" D, Uses 8 1/2" X 11" thermal Z-fold paper. q) One
(1) each 16 Channel Analog Output Computer Interface Option. r) One (1)
each CardioImage Fluoroscopy Image Management System for Satellite. s)
One (1) each CardioLab Satellite System, acquisition and review,
ClabPro-600 System Controller (as backup for CardioLab System main
computer): 180 MHZ Pentium processor, 32 MB of RAM memory, PC keyboard
and trackball, 1.2 GB high performance hard disk drive, 3.5", 1.44 MB
floppy disk drive, High performance Optical Drive II System. (vii)
Date of delivery is no later than 90 days after notice of award. Place
of delivery is Walter Reed Army Medical Center, Property Management
Division, 6825 16th St., NW; Bldg 52, Washington, DC 20307. FOB
Destination. (viii) Solicitation provision at FAR 52.212-1,
Instructions to Offerors- Commercial Items (OCT 1995) is hereby
incorporated by reference. (ix) All responsible sources may submit an
offer which shall be considered by the Government. A contract resulting
from this solicitation, will be awarded by the Government to the
responsible offeror whose offer conforming to the solicitation is most
advantageous, representing the best value to the Government, price and
other factors considered. The following factors shall be used to
evaluate offers: Evaluation will be based on (1) Technical (a)
Technical capability of the item offered to meet the Government
requirement, and (b) Delivery Terms -- ability to meet required
delivery schedule and (1) Price. Factors are equal in importance. (x)
Offeror's are reminded to include a completed copy of the provision at
52.212-3, Offeror Representations and Certifications-Commercial Items
(JAN 1997), with its offer. (xi) Clause 52.212-4, Contract Terms and
Conditions-Commercial Items (MAY 1997), is hereby incorporated by
reference. (xii) Clause 52.212- 5, Contract Terms and Conditions
Required to Implement Statues or Executive Orders-Commercial Items (AUG
1996), is hereby incorporated by reference. (xiii) Additional Contract
Terms and Conditions applicable to this procurement are: (a) Type of
Contract -- A Firm-Fixed Price contract will be awarded. (b) Period of
Performance: Delivery no later than 90 days after notice of award.
