Wolfe, 304/285-5883 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; BIDS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as Request for Proposal No. RQ97-039. This procurement is being issued unrestricted. The Standard Industrial Classification is 2399 with a size standard of 500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. The National Institute for Occupational Safety and Health (NIOSH), 1095 Wilowdale Road, Morgantown, WV plans to purchase t-shirts as part of an incentive for the merchant associates participating in the NIOSH back injury study. The associates play a very important role in collection of data related to back injury and this incentive recognizes their contributions during thetwo-year study. The contractor shall furnish custom screen printed WHITE short sleeve T-shirts, 100% cotton, higher-end quality equivalent to Hanes style #5280 or Lee style #8310. The logo will appear as a full front (approximately 8x10"). The following sizes and quantities are required: Item 001 -- 4,000 each (extra large); Item 002 -- 4,000 each (extra, extra large). OPTIONAL Item 003 -- The contractor is responsible for providing the screen for the logo "Together We Can". Offerors shall identify their requirements for printing preparation in their quote. The Government will provide a copy of the artwork and is capable of providing the separated films, if requested. Any screen, film and/or preparation costs shall be priced in your initial offer. A BID SAMPLE IS REQUIRED -- INCLUDE WITH YOUR OFFER, ONE PLAIN, WHITE T-SHIRT IDENTICAL TO THE ONE YOU WILL PROVIDE IF AWARDED A CONTRACT. The government will evaluate against the specification stated above and will also examine the bid sample to determine the quality of the textile product. The Government desires delivery of all items within four (4) weeks after receipt of award. The estimated date of contract award is September 22, 1997. Offers shall be proposed based on F.O.B. Destination (Morgantown, West Virginia). Information provided must demonstrate the unit's ability to meet the specification or your offer may be rejected. The following FAR provisions apply to this solicitation: FAR 52.212-1, Instructions to Offerors -- Commercial Items; 52.212-4, Contract Terms and Conditions -- Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items. Facsimile Bids are not authorized. Offers that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration.
