commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notices. This announcment constitutes the only solicitation; proposls
are being requested and a written solicitation will not be issued. The
solicitation number is 459-97-0030 and is issued as a request for
quotations (RFQ) under "commercial services". This solicitation
document incorporates all applicable provisions and clauses in effect
through Federal Acquisition Circular 90-46. This procurement is 100%
set aside for small business. The applicable SIC code is 7629, and the
size standard is $5.0 Million. A full equipment listing may be
obtained by e-mailing, FAXing or calling the Contracting Officers. Line
#1 is to provide Biomedical Equipment Maintenance and Repair services
for the VA Center for the Aging located on the grounds of Tripler Army
Medical Center as described in the statement of work. Range of 350-375
pieces of medical equipment, (electric/electronic/and mechanical) -- 1
year with 2 additional years at the option of the government. Initial

at the option of the government. Initial Year, October 1, 1997 through

(Health) Physicist as needed to test radiological equipment. Rate per

Optional year two $__ __ __ __ ____ . Work Statement: Provide for full
preventive maintenanceand repair services for Government-owned medical
equipment located at the Department of Veterans Affairs Center for
Aging complex located on the Tripler Army Medical Center grounds and at
the VA Medical and Regional Office Center in downtown Honolulu. Contact
the contracting officer for a full listing of equipment to be covered
under this contract. Service will include all labor, transportation,
equipment and supplies necessary to inspect, clean, repair, lubricate,
test, update and calibrate to maintain the equipment and components in
proper working order including intervening and emergency calls required
between regular inspections. Preventive Maintenance Inspections:
Preventive maintenance inspections shall be performed twice a year on
a schedule which is mutually acceptable to the Government and the
Contractor and of which is consistent with the Government's operating
requirements, manufacturers recommendations, JCAHO (Joint Commission
Accreditation of Health Care Organizations) Environment of Care
Standards and based on the specific needs of the equipment. This
service shall be scheduled with the Chief, Engineering service or his
designee. Preventive maintenance will include lubrication, adjustments
and replacements of maintenance parts as deemed necessary by the
contractor. Preventive maintenance will be performed only between the
hours of 8:00 a.m. and 4:00 p.m., Monday through Friday, excluding
holidays. Following completion of the preventive maintenance
inspection, or following any requested service call, the Contractor
will immediately inform the Chief, Engineering Service or his designee
of any unsafe equipment or any misuse or negligence incurred by users.
Regular Service: This service includes all repairs, including all
intervening calibrations occurring through normal use of the equipment
(fair wear and tear). It will also include incoming acceptance for
electrical safety inspections of all newly acquired medical equipment
in accordance NFPA 99 Chapter 7. Repairs to be made during normal
working hours, 8:00 a.m. to 4:00 p.m., Monday through Friday. Emergency
repairs will be required on weekends and holidays. Response times shall
be within four (4) hours by telephone response and with a six-hour
total on site response from the time of the placement of the service
call. All work is to be accomplished in accordance with manufacturer's
instructions including, but not limited to, adjustments, calibrations,
cleaning, lubricating, testing, disassembly, check-out, replacement of
worn and/or defective parts, etc. Service Reports: After each service
call, including preventive maintenance inspections, a service report
shall be completed in legible form and shall include the date of
service, total hours of call, description of the service performed, a
listing of any parts installed to correct a failure and any other
significant information pertinent to the service call. The service
report must be signed by the contractor's service representative and by
the Chief, Engineering Service or his designee. Parts Procurement:
Maintenance and repair parts are included in this contract but are
limited to a cost of under $50.00. Parts costing more than $50.00 will
be procured by the Department of Veterans Affairs Engineering Service
and provided to the contractor for installation. Replacement of parts
will be coordinated by the Chief, Engineering Service or his designee.
Contractor will be notified when ordered parts are received by
Engineering Service and will resume work within eight (8) hours after
notification. Contractor Qualifications: The successful contractor must
provide at the time of cost submission, a comprehensive list of all
employees who may be expected to provide service to the Government's
equipment. This list will be kept up-to-date as new personnel are hired
with respect to this contract. Service personnel who arrive on station
to provide service that are not included on this list will not be
allowed access to the equipment. Qualifications will include at least
one (1) year of experience repairing and maintaining all the types of
equipment listed in the attached equipment listing. Should the
Government request evidence of training for any contractor employee,
the successful contractor must present information relating to (a) the
location where experience was obtained, including address and
telephone number; (b) when the training was successfully completed; and
(c) copies of certificates of successful training completion. Other
Services: 1. Provide a health physicist to perform radiation protection
survey on x-ray equipment when required in accordance with applicable
