Toy, Contracting Officer, 510/803-2778 The CBD Notice, Number SN112629-0085, Laboratory Services for FCI Sheridan, Oregon is hereby modifed in its entirety to read as follows: The Federal Correctional Institution, Sheridan, Oregon has a requirement for Laboratory/Patology Services and supplies. Services will be performed at the contract laboratory. Specimens will be collected by institutional staff and shipped/transported to contract laboratory on a daily basis. Specimens will be accompanied by a request slip. Pathology and Laboratory Medicine Service will be responsible for testing most specimens on the day they are received. This contract will require various equipment and supplies be furnished to FCI Sheridan, such as vacutainer tubes, needles, specimen containers, etc. Award will be made on an "All or None" basis. The laboratory must be accredited and certified as a "Highly Complex Laboratory", and must meet all 'CLIA' and 'HCFA' regulations. The Laboratory must have a licensed Pathologist on staff. The Bureau of Prisons intends on awarding a contract with a 12 month Base Year and 4-1 year Option Periods. The anticipated Date of Award is on or about October 17, 1997. All offers received in response to this notice will be subject to the evaluation criteria as cited in the Request for Quotation.
