767-1183 R -- (i) This requirement was previously advertised as
SP4700-97-R-0031, which was cancelled 12 Aug 97. All questions
regarding this procurement shall be addressed to the contracting office
above in writing, at the following facsimile (FAX)number (703)
767-1183. No telephone calls will be accepted by this office regarding
this RFP. This is a combined synopsis/solicitation for commercial item
prepared in accordance with FAR subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. The proposal shall be
received on or before 3PM Daylight Savings Time (DST) on 11 Sep 97 in
room 1145, at the above listed address. (ii) The solicitation number is
SP4700-97-R-0032. This solicitation is issued as a request for proposal
(RFP). (iii) The solicitation Document and incorporated provisions and
clauses are those in effect through FAC 90-46. (iv) This solicitation
is not set-aside. The standard industrial classification code is 8999
and the small business size standard is 5 millions of dollars. The
small business competitiveness demonstration program does not apply.
(v) Quantity and description see Statement of Work (SOW). (vi) Delivery
date to begin 1 Oct 97. The provision at 52.212-1, instructions to
offerors-Commercial, applies to this acquisition. (vii) Under the
provision at 52.212-2, evaluation-Commercial items, best value to the
Government will be the evaluation criteria (see also SOW). (viii)
Offerors will include a completed copy of the provision at 52.212-3,
offeror representations and certifications-Commercial items, with its
offer. (ix) The clause at 52.212-4, contract terms and
conditions-Commercial items, applies to this acquisition. (x) The
clause at 52.212-5, contract terms and conditions required to implement
statutes of executive orders-Commercial items, applies to this
acquisition. (xi) SOW (in part) C.2, Scope -- DCMC has approximately
15,000 military and DoD civilian personnel located within three (3)
districts. The contractor shall provide a comprehensive childcare,
family and personal issue 24 hour, seven (7) day a week, toll free
telephone and referral service, as well as accessibility via Internet
resource, as defined in section C.3, to participating DLA and DCMC
components nationwide. These services shall be at no cost to the
applicable Government employees. C.2.2. Potential exists to expand the
contract to other organizations and agencies as the need may arise.
Further, this effort will cover one (1) base year (FY98) with the
possibility of up to four (4) option years. C.5. Deliverables -- The
contractor will deliver the following items, in the quantity within the
time specified as follows: (1) Sample promotional information, 3
copies, 7 calendar days after contract award; (2) Marketing/Promotional
materials, the contract award in envelopes addressed to each employee,
separated by either inter-office mail or outgoing mail, 15,000 copies,
30 calendar days after contract award; (3) Lunchtime Seminar Evaluation
Questionnaire, 1 copy, 30 calendar days after contract award (4)
Customer Satisfaction Questionnaire for resource and referral issues,
1 copy, 90 days after contract award: (5) Monthly activity reports, 2
copies, 15 calendar days after end of month; (6) Quarterly activity
reports, 2 copies, 15 calendar days after the 3rd, 6th, 9th, and 12th
month of the contract; (7) Semi-annual activity reports, 2 copies, 15
calendar days after the end of the 6th month of the contract; (8)
Annual Program Evaluation and Annual Activity Report, 2 copies, 15
calendar days after the end of the 12 months of the basic ordering
period; Seminars at each of two (2) designated regions, four (4) to be
determined with the COR (for a total # of seminars, eight (8).
