Code DADA16) The Tripler Army Medical Center (TAMC), Honolulu, HI
intends to award a contract for a Material Handler to serve as Tripler
Army Medical Center's Forms/Records (to include medical records)
Warehouse Worker and Records Holding Area Custodian for the period
October 1, 1997 through September 30, 1998 with an option to renew for
three additional one year periods. This isa combined synopsis for
commercial services prepared in accordance with subparts in 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. This
solicitation is issued as a request for quotation. The solicitation and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 90-46. This procurement is 100% set aside for
Small Business. Tripler Army Medical Center intends to contract for the
following services Monday through Friday 7:30 a.m. to 4:00 p.m.,
excluding federal holidays. 1. Receives and processes DA Form 17 from
Admin Forms Manager. 2. Requisitions, receives, stores, and issues
local publications and blank forms required by TAMC units and
activities. 3. Establishes operational procedures and guidelines for
efficient functioning of the branch and makes adjustments as necessary.
4. Reviews, interprets, and applies regulations, policies and other
command directives affecting internal operations. 5. Responsible for
filling request and calling requesting customer for pickup of order. 6.
Insures the safeguard of controlled and sensitive forms. 7. Enforces
operational decisions for the Publications Branch regarding
procurement, storage and distribution of publications and blank forms.
8. Advises the Contracting Officer Representative of major changes or
those of a controversial nature concerning the functioning of the
Branch, providing recommendation(s) for appropriate course(s) of
action. 9. Reviews and analyzes demands or requirements for TAMC
publications and blank forms and initiates appropriate action to assure
that stockage is adequate to meet the needs of serviced units and
activities. 10. Initiates and coordinates requests to print additional
stockage of publications and forms. 11. Responsible for activities
regarding maintenance of stock records cards, preparation and review of
requisitions, accomplishing inventory adjustments, processing incoming
shipments, filling requisitions, packing and shipping, warehousing and
rewarehousing, restocking, and salvaging obsolete materials. 12.
Maintains a backup area in the same location of 120 day stock of forms.
13. Processes approved requests for publications or blank forms
submitted by serviced units and activities. 14. Serves as advisor to
superior and serviced units and activities regarding all aspects of
issuance of local publications and blank forms. 15. Insures compliance
with TAMC fire prevention and safety precautions, taking necessary
actions to eliminate hazards which endanger life and property. 16.
Assistthe TAMC Records Management Office in the Retiring of TAMC
Records to include Medical Records and the storage of records. 17.
Coordinates shipment of Records with Fort Shafter Transportation and
Hickam Air Force Base. 18. Contractor shall insure records are placed
in shipping containers properly based on assigned assession number
received from the Federal Records Center. 19. Contractor must be
familiar with Records Transmittal (SF 135) and Transportation Form (DD
Form 1384). 20. Responsible for banding all shipment containers with
steel banding equipment. 21. Must be familiar with Regulations that
governs records such as Army Regulation (AR) 40-66 (Treatment Records)
and AR 25-400-2 (Modern Army Records Keeping System). 22. Contractor
shall on an average be responsible for shipping out a total of 1,000
boxes of records per year and the storage of 1,500 boxes of records.
23. Operates a government vehicle to make trips to the Publication
Center at Schofield Barracks East Range to deliver requisitions and
pickup publications and blank forms on an as needed basis. These trips
shall normally be associated with regular requisition, special
requisitions received from TAMC units or activities which sometime
requires delivery. 24. Make special trips to pickup GSA items from
local GSA stores. 25. Utilizes fork lift equipment to move pallets of
forms, books, regulations, records, boxes of paper and other items that
will need material handler support. 26. Frequently lifts boxed items
weighing up to 40 pounds. 27. The Contractor plans own work and follows
established procedures, keeping COR informed of progress and/or problem
areas encountered. 28. Work is reviewed for adequacy of planning,
appropriateness of methods/techniques used. 29. The Contractor is
relied upon to make recommendations and to recognize and refer to COR
those questions which are beyond the scope of his/her assigned
responsibility. This is a "Nonpersonal Services Contract" as defined in
FAR 37.101. All offers will be evaluated and considered in
accordancewith the following evaluation factors: 1. Technical
Experience. 2. Past Performance. 3. Price. The offeror will provide
documentation of the three evaluation factors. The proposal must
include a current resume documenting at least 2 years experience in the
operation of Records/Medical Records Storage and the preparation of
Records/Medical Records for shipment. The Contractor shall posses a
valid drivers license and be qualified to operate a forklift up to 6K
in weight. Must be able to type a minimum of 25 wpm. The Contractor
shall read, understand, fluently speak, and legibly write the English
language. These factors are listed according to their importance with
number 1 being the most important, then #2 and #3. The Government will
award a contract resulting from this solicitation to the responsible
offeror whose offer conforming to the solicitation will be most
advantageous to the government, price and other factors considered. The
Government reserves the right to award a contract to other than the
lowest offeror or not to make an award if it is deemed to serve its
best interest. The Contractor is required to meet all
health/immunizations requirements prior to the start of the contract
and comply with all policies pertaining to Substance Abuse, Security,
Attendance and Personal Appearance. The following provisions and
clauses apply to this acquisition. These clauses and provisions are
