A. -- INTRODUCTION: The Social Security Administration is interestedin receiving quotations from capable firms that can provide 1,600sets of 35 color transparencies produced from 35 mm slides thatwill be furnished by the Government. This is a combined synopsis/request for quotation numbered SSA-RFQ-97-3353 for commercial item supplies/services prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. THIS IS THE ONLY ANNOUNCEMENT. A SEPARATE WRITTEN REQUEST FOR QUOTATION WILL NOT BE ISSUED. This procurement will be conducted as a simplified acquisition procedure in accordance with FAR Part 13. The standard industrial classification code for thisacquisition is 7334. This request for quotation and incoporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. Quotations in response to this request for quotation shall be submitted by September 8, 1997, 3:00 P.M.,Eastern Standard Time, to the address provided herein to the attention of Wayne McDonald. B. -- REQUIREMENT: A qualified contractor will provide 1,600 sets of 35 color transparencies produced from 35 mm slides that will be furnished by the Government. The Government will review one set of transparencies with slip sheets prior to mass duplication. Sets of 35 transparencies with slip sheets plus 8 page script are to be packaged in a box with paper copies of the transparencies. The box is to be permanent storage for transparencies. The contractor will insert boxes into envelopes using Government postage indica. Viewgraphs (8"x10") are to be mounted with correct positioning in heavy white double paper mounts, imprinted with titles, numberedand collated. The contractor will be supplied with mailing lables for the shipment of the 1,600 sets of 35 color transparencies. The contractor will be required to have regular communication with theGovernment during the development stage of the transparencies and coordinate receipt of the Government's 1998 legislative number changes. The following provisions at FAR 52.212-1, Instructionsto Offerors -- Commercial, (tailored) apply to this acquisition:(b)(1), (b)(3) & (b)(4), (b)(6), (b) (8) and (b)(11), (c), (d), and (g). There are no addenda to these provisions. The provisions at FAR Part 52.212-2, Evaluation -- Commercial Items apply to thisacquisition. The government will award a purchase order resulting from this request for quotation to the responsible offeror whose offer conforming to the quotation will be most advantageous to the Government, price and other factors considered. The followingfactors shall be used to evaluate offerors in descending order ofimportance: lowest price and experience and past performance. Offerors shall submit firm fixed prices for the supplies/servicesrequired to be paid in arrears after delivery of the transparenciesOfferors shall submit samples of color transparencies previouslyproduced from other clients to demonstrate experience that isrelevant and similar to this requirement. Offerors shall submitreferences from at least 3 federal government agencies that SSAwill contact to determine the customer's satisfaction with theofferor's past performance. Offerors are advised to includea completed copy of the provision at FAR Part 52.212-3, OfferorRepresentations and Certifications -- Commercial Items, with theiroffer (tailored). The provisions at FAR Part 52.212-4, ContractTerms and Conditions -- Commercial Items apply to this acquisition. There are no addenda to these provisions.
